Transitional Services Coordinator

  • Monitors the development, implementation, and documentation of individual plans for all individuals in Transition Services.
  • Oversees all documentation and materials specific to Stakeholder’s Standards. (DDS, CARF, BRS)
  • Attends meetings and case reviews as assigned.
  • Provides trainings and in-services to Community Inclusion Specialists to ensure accountability for the efficient implementation of consumer’s programs.
  • Conduct interest inventories and assessments to establish an individual’s career goals.
  • Conduct various career exploration activities with program participants including job shadowing, informational interviews and volunteer experiences.
  • Conduct various career preparation activities with program participants including resume writing, video portfolio, application assistance, mock interviews, transportation training, and professional image review. Benefit analysis and review to determine impact of future earned wages.
  • Financial literacy skill building.
  • Facilitate groups (Social Skills, Travel Training, Anger Management, etc)
  • Research, develop, and implement individualized habilitative, vocational and volunteer options for individuals in program.
  • Initiates and maintains ongoing contacts with a variety of business and industry representatives to develop volunteer, Internship, and/or job sites.
  • Perform other related duties as required.

Educational Background: 
Associates Degree in Human Services or related field required. Bachelor’s degree in related field strongly preferred.
Skills/Experience: 
  • Familiar with goal-directed, outcome-oriented work in a vocational habilitation and rehabilitation environment.
  • Excellent computer skills, i.e. Word, Excel, PowerPoint, Email.
  • Work effectively in collaboration with diverse groups of people.
  • Excellent problem-solving skills.
  • Clear and effective communication skills.
  • Display good sound judgement.
  • This position may be authorized to have access to protected Health Information (PHI) to perform specific job duties. 
  • Valid driver’s license, good driving record, personal vehicle and active insurance.
  • A minimum of three (3) years of work experience providing employment/vocational services to the target population and/or persons with similar needs required.
