Summary:

The Direct Care Professional is responsible for participant progress toward Individualized Service Plan (ISP) goals and objectives and must understand and implement ongoing “active training” in an assigned group home location (2 - 6 participants) within our ICF/MR and DD Waiver programs.  The Direct Care Professional provides direct training and other support services to participants, at one or more locations, and documents such as specified in the Individualized Service Plan (ISP) while providing community integration activities at all times. Support services to participants include but are not limited to: personal hygiene, housekeeping, cooking, following behavioral, medical, and Individualized Service Plans to provide a healthy, safe, and meaningful environment to our program participants.

Qualifications:
  1. High School Diploma or GED with accreditation as determined by the State of New Mexico. Must be able to provide proof (original diploma or official transcripts) upon hire.
  2. Must be able to work flexible hours/locations.  Regular attendance is a requirement of the position.
  3. Valid New Mexico Driver’s License issued more than three years ago with no DWIs/DUIs and no more than three moving violations within the last three years.
  4. Must obtain CPR/First Aid certification within first 30 days of employment and maintain during employment (ESEM offers training during orientation).
  5. Current negative TB skin test results (within last year).
  6. Current drug screen with negative results.
  7. Must have current telephone or message phone.
  8. Must pass NM Caregivers Criminal History Screening.
