- Facilitates person center planning and child & family team meetings.
- Develops initial and ongoing revisions of person-centered plan and monitor the implementation of person-centered plan.
- Supervises and monitors service delivery through home and community visits assuring services meet or exceed state, local and ESUCP standards.
- Assists Licensing Coordinator with recruitment and provision of training of prospective foster parents.
- Provides direct supervision and support to Therapeutic Foster Families.
- Visits each Treatment Family weekly to provide face to face supervision/guidance and to overs treatment.
- Participates in on-call rotation for routine and emergency situations.
- Responds to emergency/crisis situations as needed during the day, night, holidays and weekends.
- Performs other duties as assigned by supervisor.
Educational Background:
Bachelor’s degree in social work from an approved, accredited school of social work; or Bachelor’s degree in a human services field from an approved, accredited college or university plus one year directly related experience
or Bachelor’s degree in a non-human service related field from an approved, accredited college or university plus two years directly related experience.
Skills/Experience:
- Directly related experience is defined as human services experience in the areas of case management, assessment and referral, supportive counseling, intervention, psycho-social therapy and treatment planning.
- Valid unrestricted, state appropriate driver’s license and ability to be insured by Easterseals UCP’s insurance carrier.
- Must have reliable transportation and the ability to transport self.
