Under the supervision of the Manager of Community Services the Individualized Support Specialist are primary implementers of our organization’s services, working on the front lines in our communities.  As our professional representatives in the field, we view their roles as particularly critical to the overall success of the programs we provide.  The Individualized Support Specialist provides comprehensive direct supports that empowers individuals to be as independent and successful in their vocational goals and/or life skills.  The Individual Support Specialist provides hands-on support in the instruction and development of vocational and/or daily living skills based upon the individual’s need. The Individualized Support Specialist provides a range of services for individuals based on their needs in their home and/or community. This includes but is not limited to personal care, domestic house hold services, personal appointments, medical appointments, vocational services, community activities, supervision within the home, and other task based upon their individual goals.

Areas of Responsibility: 
  • Provides face to face hands on supportive assistance enabling individuals to maintain living in their home and community including activities of daily living, personal care and other activities to promote healthy living.
  • Assumes responsibility for implementation of individualized treatment plan goals, teaching strategies, and reporting assignments.
  • Establishes, develops and maintains respectful relationships with individuals served, and facilitate functional training in vocational, domestic, recreational, leisure and community settings.
  • Provides on-site employment and life skills training. Maintains on-site support to consumers and serves in a liaison role with employers per their individual goals.
  • Assists individuals in accessing public and/or generic transportation (native supports); provides transportation as assigned.
  • Assists/educates with personal care such as eating, toileting, bathing and dressing as prescribed, based on the needs of the individuals served.
  • Assists/educates individual with household chores as needed.
  • Assists individuals in making personal choices and accessing community activities.
  • Implements Behavior Support plan and required guidelines.
  • Implements medical/health protocols and required guidelines.
  • Teaches and enforces safety practices.
  • Provides vital input to the development of the individual’s Individual/Treatment Plan.
  • Encourages the use of natural supports and fosters involvement in social and community activities while maintaining safety of the individuals supported.
  • Supports individuals to engage in routine medical, psychological, dental appointments, personal business and other appointments as needed.
  • Provides individual access to community resources through transportation in personal vehicle with destinations pre-approved and responsible management of the mileage budget as determined and directed by the Coordinator and/or Manager.
  • Assumes responsibility for implementation of individualized treatment plan goals, teaching strategies, and life goals.
  • Actively participates as an advocate of interdisciplinary treatment teams for all assigned individuals.
  • Establishes, develops and maintains respectful relationships with individuals served, and facilitate functional training in vocational, domestic, recreational, leisure and community settings.
  • Completes all required documentation in Therap (an electronic documentation system) each shift and other methods of documentation as needed i.e. monthly mileage reports, incident reports, etc.
  • Documents data for the individual’s goals, ADL strategies, behavior tracking, and all other forms of documentation as requested and needed within Therap or other electronic systems as directed.
  • Maintains and attends all required agency, state, or accreditation trainings as assigned including but not limited to PMT, CPR/First Aid, Abuse/Neglect, CODS, and Cornerstone.
  • Attend team department meetings and supervision with Community Services Manager
  • Establishes rapport and working relationship with individuals being supported and if applicable, family residing in home
  • Demonstrates sensitivity and commitment to the individuals supported and family members within the home if applicable
  • Perform other related duties as required.

Educational Background: 
High School diploma or equivalent required. Associates Degree Preferred.    
Skills/Experience: 
  • Does not contribute to clutter or uncleanliness of family/individual home.
  • Must have smart phone capable of completing electronic documentation shift notes in Therap, Outlook, ADP for time and attendance, EVV and other work related applications as required (i.e. Skype, Facetime for communication with your manager)
  • Effectively and clearly communicate to individuals served in a supportive tone.
  • Position may require 1st, 2nd, overnight and weekend shifts.
  • Valid Connecticut Driver’s license, current car insurance and registration with clean driving record required.
  • Must be flexible to work at various locations with individuals in their home setting.
  • Personal vehicle is required for transportation of individuals served.
  • Able to manage and maintain a delegated budget for services delivered (i.e. mileage, recreational budget).
  • Sign-off of personal vehicle policy and supplemental documents required.
  • This position may be authorized to have access to protected Health Information (PHI) to perform specific job duties.
  • One to three years of experience in human services or relevant field.  In home support experience preferred.
