A Rehab Coordinator is a Direct Support Professional who assists clients working in our Mt. Vernon IN workshop and as part of our mobile work crew at SABIC. Typical work hours are 7am-4pm. $11.00 per hour.
Areas of Responsibility:
- Maintain strict confidentiality regarding client, employee & agency information.
- Observe & assist consumers with their work &/or opportunity time activities. Provide verbal direction to consumers & part time workers.
- Complete & submit data sheets.
- Complete chrono sheets & incident reports.
- Assist consumers with break & lunch activities.
- Drive van routes as assigned.
- Maintain Transportation Recipient Service & Habilitation Training logs.
- Work at SABIC &/or work center as assigned.
- Assist in ensuring industrial safety & rules compliance.
- Report data & important consumer information.
- Log consumer attendance.
- Perform vehicle inspection check lists, mileage, fuel purchases.
- Load/unload trucks.
- Participate monthly tornado/fire drills.
- Participate in SABIC weekend rotation.
- Perform light maintenance on facility & vehicles.
- Assist with production needs.
- Assist with submission of required data.
- Inform Work Center Case Manager of consumer successes, challenges & issues.
- Advocate for needed services for consumers & make referrals as needed.
- Attend all department meetings possible & report on areas of service & need.
- Deal with consumer behaviors & advise/support production staff, report & discuss solutions.
- Maintain teamwork effort with others on production staff & outside Group Home staffs.
- Other duties as deemed necessary.
- Look for ways to be of service to consumers & co-workers.
- Maintain a hazard free work area.
Skills/Experience:
- Must be reliable, motivated, compassionate.
- Must pass a pre-employment physical, drug screen, & TB test.
- High School diploma/GED
- Experience in direct care desired but not required.
- Must be at least 18 years old.
- Valid driver's license required.
PHYSICAL REQUIREMENTS:
- Ability to assist clients in activities requiring kneeling, crouching, reaching, and bending.
- Ability to lift & transfer, with assistance, individuals who weight up to 250 pounds.
- Ability to push large adults in wheelchairs.
- Ability to lift 70 pounds occasionally and 40 pounds routinely.
