Basic Responsibilities: • Provides case management for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team. • Assumes primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and Person Centered Plans in conjunction with the Individualized Treatment Team (ITT). • Provides individual supportive therapy and system management. • Educates and supports client’s families; and advocates for clients’ rights and preferences. • Participates in comprehensive assessment of psychiatric history (e.g., onset, course and effect of illness, past treatment and responses and risk behaviors), mental status, and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living (e.g. self-care, living situation, nutrition, money management); and family structure and relationships. • Consults with community agencies and families to maintain coordination in the treatment process. Provides on-call intervention covering nighttime hours and serves as a backup to evening and weekend staff. • Participate in 24/7 on call crisis phone rotation. • Documents client progress to maintain a permanent record of client activity according to established methods and procedures. • Participates in daily staff organizational meetings and treatment planning review meetings. • Provides ongoing assessment of clients’ mental illness symptoms and clients’ response to treatment. • Provides symptom education to enable clients to identify their mental illness symptoms. • Psychosocial rehabilitative interventions and case coordination. • Complete required trainings within prescribed timeframes including the approved ACT Team training, including the completion of the DHHS approved Tenancy Support training Qualifications: • A Bachelor’s degree in a human service field with at least two-year post-graduate experience working with adults diagnosed with severe and persistent mental illness • Must be a Qualified Professional (QP) • Candidate must possess excellent oral and written communication skills • Must have a valid, unrestricted, state appropriate driver’s license for the State of North Carolina. • Preferred: Experience providing tenancy support and familiarity with housing programs.Job Function:
