About our Medical Rehabilitation Services: Our services include physical therapy, occupational therapy, and speech and language pathology provided by a team of licensed, registered healthcare professionals who work together to assess and meet the physical needs of each individual with the goal of rehabilitating the whole person.

Under the supervision of the Site Director and supervision provided by the Medical Director, provides high quality patient care through evaluation and treatment of patients as prescribed by a primary care provider. Services include evaluations, assessments, design of treatment programs, implementation and supervision of treatment programs and educational services.

Areas of Responsibility: 
  • Evaluation and treatment of patients relative to their physical therapy needs. Demonstrating critical thinking and problem solving skills to determine the need of physical therapy services.
  • Formulation of evidence based treatment plans and goals for each patient, relative to their physical therapy needs.
  • With physician approval, implementation of the physical therapy services defined in the patient care plans.
  • Ensure completion of all required documentation of patient care activities; evaluations, daily notes, progress/monthly notes and discharge notes reflecting clinical reasoning skills.
  • Participate in plan of treatment reviews, patient case review conferences, patient assessment and reassessments, outside surveys as appropriate.
  • Completing billing in a timely manner.
  • Effectively interact and convey the plan of care to referral sources and insurance/payor sources.
  • Supervise, as needed, the administration of physical therapy services by physical therapy assistants and/or physical therapy students as needed.
  • Provide necessary educational services to students, staff, and public as assigned. Participates in educational programming by attending inservices.
  • Maintain proper credentialing and continuing education credits required for licensing.
  • Perform other related duties as required.

Skills/Experience: 
  • This position is authorized to have access to protected Health Information (PHI) to perform specific job duties. 
  • Excellent communications skills with patients, families and team members. Displays an even temperament and maturity in judgement.
  • Participates as a productive member of the Team. Displays flexibility, a willingness to assist others, cooperation, and respect for others.
  • Attends departmental and other meetings as required.
  • Participates in Continuous Quality Improvement activities; as directed in the following, but not limited to: Utilization review, Quality Assurance, Program Evaluation, Infection Control, Health and Safety Committee Activities and Chart Reviews and contributes to program development.
  • Works towards budgeted benchmarks, established by the budget for average daily charge (ADC) and visits per day per therapist (VPDPT).
  • Identifies program needs and reports these to the Site Director.
