Under direction of the CLASS Program Director together with the Vice President of Community Living Services, the CLASS Case Manager provides comprehensive case management services to individuals enrolled in the Community Living Assistance Support Services (CLASS) Medicaid Waiver program, administered by the Texas Department of Aging and Disability Services (DADS).  The CLASS Case Manager consistently meets all CLASS program standards as stated in the CLASS Provider Manual, Policy Revisions/Clarifications, and Information Letter Clarifications.  The CLASS Case Manager is an advocate for the clients assigned to his/her caseload, uses the help of the CLASS Administrative Assistant appropriately, and seeks guidance and clarification from the CLASS Program Director as needed and with a commitment to consistent and quality care for all clients.

Although it is not possible to provide an exhaustive list of all types of duties, responsibilities and performance standards, the following are essential job functions:

General

  1. Be at work on time and ready to start work.
  2. Regular attendance required during regular business hours; is subject to supervision during working hours; requires interactions with coworkers and supervisors.
  3. Participates in Easterseals North Texas meetings as required and appropriate.
  4. Knows and abides by Easterseals North Texas policies and procedures.
  5. Promotes a unified, team-oriented atmosphere in all communications and actions.
  6. Is professional in appearance and demeanor.
  7. Contributes to providing quality care for clients of Easterseals North Texas in whatever capacity is required in accordance with skills and training.
  8. Maintains awareness of trends and best practices, and is proactive in continually acquiring knowledge in fields related to disabilities, rehabilitation, case management, and advocacy.

 

Service Delivery Responsibilities

  1. Performs all required case management functions within specified timeframes.
  2. Maintains full, accurate and current documentation of evaluations, assessments, needs, progress, services, financial data and all other categories of information required in each individual’s record.
  3. Understands and subscribes to the philosophy and values of independent living, empowerment and community integration of all participants in the CLASS program, and demonstrates same in all communications and actions.
  4. Empowers CLASS clients to make informed decisions and supports clients’ decisions regarding important aspects of life, such as housing, employment, relationships, education and other personal goals.
  5. Knows each client in his/her caseload personally and is familiar with their goals, needs, opportunities and challenges.
  6. Meets regularly (no less than monthly) with clients and/or allies to discuss progress, problems, and plans and to monitor services provided according to the clients’ Individual Services Plan (ISP).  Whenever possible, such contacts will occur where the participant lives, works, attends classes or plays.
  7. Maintains cooperative working relationships with Direct Service Agencies (DSAs) and their representatives, including regular contact and consultation.  This cooperation will enhance service delivery and assist the participant in achieving his/her individual goals.
  8. Responds to telephone calls in a timely fashion, and always within 24 hours.  Ensures clients have access to Case Management in emergency situations.  Responsible for assuring phone coverage when unavailable.
  9. Completes monthly program reports and program billing forms and turns in to CLASS Program Director as required.
  10. Remains updated and knowledgeable about community resources and state and federal programs as they relate to CLASS client needs.

 

Community and Public Relations and Development

  1. Assists with any assigned tasks/responsibilities relevant to the Affiliate’s comprehensive development plan and activities.
  2. Contributes to building Easterseals’ brand equity, including the ability to clearly articulate the Easterseals North Texas mission, vision and values.
Qualifications:

Education:

  • Bachelor’s degree in social work or related field is preferred.
  • High school diploma or GED is required.

Experience:

  • Must have a minimum of two years documentable experience providing services to children and/or adults with disabilities.

Skills/Abilities: 

  • Must have working knowledge of Windows Operating System and Office products, including Microsoft Word, Excel and Outlook, or the ability to learn them expeditiously and independently.
  • Must also have ability to learn to use Easterseals’ Client Database expeditiously and independently.
  • Spanish/English proficiency preferred.
  • Demonstrate strong written and oral communication skills.
  • Demonstrate an ability to work independently with a minimum of supervision.
  • Use analytical and critical thinking skills to create client plans.
  • Perform basic mathematical computations at a minimum sixth grade level.

Physical limitations: 

  • Work at a desk/computer a minimum of six hours per day
  • Frequent sitting; intermittent walking, standing, bending, stooping, kneeling, lifting up to 25 lbs
  • Must be able to efficiently and independently travel throughout the service area

Job Requirements:

  • Must be able to pass a criminal background check and drug screening.
  • Current driver’s license and proof of automobile insurance.
  • Travel throughout North Texas area to visit clients, both in Dallas and Fort Worth area. 
Additional Notes: WORKING CONDITIONS AND HOURS: Primarily office conditions, however local travel throughout both Dallas and Fort Worth is required. General working hours are 8am - 5pm with 30-minute lunch breaks. Some additional evening, early morning and weekend work may be required with this position. Flexibility in hours will be determined in consultation with supervisor.
