Become an important part of our Community Living Services team. CLS enables individuals with intellectual & developmental disabilities to thrive in a home environment. Services are provided based on each person's unique needs & goals.

Areas of Responsibility: 
  • Maintain strict confidentiality regarding client, employee & agency information.
  • Immediately report any suspected allegations of abuse (verbal, neglect, physical, sexual) to Coordinator.
  • Complete incident reports within FSSA/BDDS guidelines.
  • Attend weekly QDDP meeting; discuss pertinent information with administration.
  • Meet weekly with coordinator to discuss/troubleshoot issues.
  • Work approx. 15-20 hours per week on floor as a DSP (fewer when on-call).
  • Rotate on-call phone with other QDDPs/Managers. Calls must be returned within 10 minutes.
  • Supervise, evaluate, schedule & train DSPs.
  • Monitor safety of clients assigned. 
  • Provide input to nursing staff to complete High Risk Management, Dining, Seizure, Side Effects Monitoring, Medication Administration.
  • Visit each site under your purview once per week; spend a minimum of ten hours per week in client homes.
  • Complete monthly summaries & attend client meetings for assigned clients.
  • Keep client demographic information accurate & current in Provide.
  • Write goals & service objectives at least annually or as ISPs change/update.
  • Serve as liaison to ensure recommendations for services & equipment are completed.
  • Ensure clients' financial requests are handled promptly & effectively.
  • Be aware of client allotted hours to facilitate good client care.
  • Facilitate admissions, discharge or internal moves of clients with all relevant parties.
  • Ensure coordination & consistency of internal & external services for each client.
  • Prepare, assist, or supervise clients with: Meal preparation, bathing, daily housework, shopping, making/attending medical appointments, medication administration, activities specified in ISP, feeding, dressing, mobility, independence with all daily living skills.
  • Scheduling: Follow times & directions on schedule; Employees do not have a set schedule; Work varied days & hours based on client needs.
  • Transport clients to appointments, shopping & recreational activities.
  • Implement behavior improvement plans by following outlines & properly documenting incidents.
  • Keep daily client progress notes through narrative notes in Provide. 
  • Adhere to all state & agency regulations & guidelines.
  • Participate in staff meetings, client meetings & in-service training.
  • Contact supervisor & find own replacement at supervisor’s discretion.
  • Enter work into Provide accurately & within 24 hours of each work segment.
  • Train new staff annually on clients’ ISPs & risk plans.  
  • Verbally test staff’s competence routinely in clients’ risk plans, prohibited interventions, Abuse/Neglect/Exploitation policy, & reportable incidents.
  • Train new staff on medication administration protocols.
  • NOTE: This position description is not an exhaustive list of responsibilities; more specifics are available from the Easterseals Rehabilitation Center. Other duties may be assigned.
Educational Background: 
Have a Bachelor’s Degree, high school diploma, or GED.
Skills/Experience: 
  • Physical lifting and/or transferring of certain clients is required.
  • Must be able to lift 40 pounds frequently and 80 pounds occasionally.
  • Have/Maintain First Aid and CPR certification.
  • Complete State Mandatory Medication Administration Training upon being hired. 
  • Have at least one year of experience working with persons with developmental disabilities or complete additional training upon being hired. 
  • Have a valid driver’s license and insured vehicle. 
  • Be at least 18 years of age.  Requires the ability to hear, see, talk, read, and write on constant basis.
