Individual in this position provides direct services to participants by greeting individuals assigned to Community Link and determining intensity of services needed through intensive interviewing, observations, self-report, standardized test results and reports from other professionals and assists participants in making the best possible work assignment decisions, making assignments to work activity to be incorporated into the community Link Case Plan, referring qualified applicants to approved training and/or other available services in accordance with their Case Plan, ensuring linkage with Work Site Coordinator. Completes necessary documentation regarding assessment, referral and other follow-up services, attends joint case conferences as necessary. Must be willing to meet individuals served out in the community. Must have knowledge of Federal welfare-to-work regulations and/or CCMEP regulations
- Knowledge of customer service principles, case management and human services principles
- Knowledge of computer software, applications and programs
- Skilled in verbal and written communication
- Ability to exhibit flexibility in work schedule & job tasks
- Exceptional organizational, planning, scheduling & problem solving skills
- Ability to maintain good public relations
- Valid driver’s license & insurance, a good driving record & reliable transportation
