Combine direct participant services with case management tasks; assure that intervention fidelity is maintained; participant evaluation and monitoring activities. Assist Agency with Home and Community Based Services and Harm Reduction services billable to Medicaid; explain interventions to stakeholders. This position reports to the Program Manager.
Areas of Responsibility:
- Implement the Harm Reduction Behavioral Health Initiative at SACHR.
- Provide Individual & Group behavioral health interventions (including intake, psychosocial [re]assessments) to participants of all departments, i.e., Harm Reduction/Syringe Access Program, Women’s Services Program, and Ryan White Program.
- Provide clinical assistance/supervision to the Ryan White Part A - Supportive Counseling & Family Stabilization Program staff.
- Uphold SACHR’s policy and procedures including the policies and procedures set forth by the NYS Department of Health and the NYC Department of Health & Mental Hygiene regarding behavioral health interventions including HIPAA requirements.
- Complete monthly participant chart reviews; follow-up (as needed).
- Provide Clinical Supervision to LMSW or Mental Health Counselor.
- Be a Field Instructor to Social Work interns.
- Assist with all required reporting on Mental Health Programs.
- Assist with implementation and provision of Home and Community Based Services and Harm Reduction services billable to Medicaid.
- Perform and oversee data entry re. behavioral health interventions in the Electronic Health Record, TREAT.
- Organize & chair regular or as needed case conferences.
- Train all staff in basic behavioral health skills.
- Represent SACHR at Community, Professional and/or Coalition meetings.
- Participate in regular supervision, training, and staff meetings.
- Perform other duties as assigned.
Educational Background:
Licensed Clinical Social Worker; or Licensed Psychologist
Skills/Experience:
- Strong verbal and written communication skills.
- Two or more years of clinical experience providing behavioral health services to vulnerable populations.
- Demonstrated understanding of social issues related to HIV/AIDS, comfortable and competent in social science research; interest in complementary therapies.
- Detail oriented with a proven record of meeting deadlines.
- Bilingual English/Spanish strongly preferred.
