The Licensed Practical Nurse integral member of the program. This position is responsible for Health Services and Care Plan Management within the center.

Areas of Responsibility: 

Health Services

  • Assess individual participant health needs and act accordingly, including administering daily medications and other skilled nursing care as needed.
  • Ensure compliance with all physician orders: follow up on returned Physician Summaries, health care provider visits, unplanned emergency department or urgent care visits, hospital and skilled nursing facility discharges and medication and treatment changes as reported on documentation provided by care giver/health care provider.
  • Coordinate and advocate with family, caregivers, and physicians for participant healthcare needs. Follow up on health-related concerns, including mandatory reporting to Adult Protective Services when signs or symptoms of passive or active neglect or abuse is suspected or identified.
  • Provide emergency triage and first aid, communicating and coordinating related care as indicated with health care providers, family and caregivers.
  • Track dietary changes for medical reasons, monitor “Dietary Considerations” for accuracy and inform all stakeholders.
  • Ensure vital signs and weights are being performed as ordered by physician for each participant and documentation is completed accurately and in a timely manner.
  • Document participant issues, goals and services provided in an accurate and timely manner.
  • Work in conjunction with the assigned Registered Nurse (RN) to ensure nursing best practices are maintained (through weekly meetings, reviews of documentation, and nursing assessments conducted) for compliance with WAC requirements
  • Participate in staff meetings, and provide health and safety trainings as need for staff.
  • Oversee CNA in performance of health service tasks. Provide CNA with training and monitoring as needed.
  • Provide assistance and guidance to CNA and program staff as it relates to participant personal care needs, including toileting and transfers
  • Perform other duties as assigned

Care Planning

  • Participate in the development of individualized care plan
  • Plan, schedule and facilitate care plan meetings with center staff to ensure Care Plan issues and goals are appropriate, that issues and goals are being addressed, and that reporting is accurate and per WAC requirements
  • Review and document ADC participants care plans quarterly and PRN if changes occur
  • Follow Easterseals Washington Health Services Policies and Procedures for care planning documentation and follow through
  • Document changes on Care Plans per Physician Summaries and notify stakeholders
Educational Background: 
Current LPN accreditation/licensure in WA State
Skills/Experience: 
  • Experience in mental health, geriatrics, and/or developmental disabilities
  • Practical working knowledge of the Washington Administrative Code for Adult Day Health (preferred).
  • Ability to work effectively in a team-focused environment.
  • Ability to prioritize and handle multiple tasks in a timely manner.
  • Strong written and verbal communication skills.
  • Proficiency in Microsoft Office and patient database programs.
  • Ability to move, lift and transfer participants using proper transfer techniques.
  • Flexible schedule of availability.
  • Current CPR/First Aid Certification, TB Testing and Food Handler’s Permit within 30 days of hire.
  • Must pass DSHS background Check (and FBI fingerprint check if a Washington resident for less than three (3) years.
  • Proof of U.S. Citizenship or right to work.
