Summary:

The Registered Nurse (RN) is an integral member of the Health Service team. This position is responsible for Health Services Management, Care Plan Management, onsite consultation with the Licensed Practical Nurses (LPN), and nursing assessments.

Basic Responsibilities:Health Services Management:
  • Assess individual participant health needs and act accordingly, including administering daily medications and other skilled nursing care as needed.
  • Ensure compliance with all physician orders: follow up on returned Physician Summaries, health care provider visits, unplanned emergency department or urgent care visits, hospital and skilled nursing facility discharges and medication and treatment changes as reported on documentation provided by care giver/health care provider.
  • Coordinate and advocate with family, caregivers, and physicians for participant healthcare needs. Follow up on health-related concerns, including mandatory reporting to Adult Protective Services when signs or symptoms of passive or active neglect or abuse is suspected or identified.
  • Provide emergency triage and first aid, communicating and coordinating related care as indicated with health care providers, family and caregivers.
  • Track dietary changes for medical reasons, monitor “Dietary Considerations” for accuracy and inform all stakeholders.
  • Ensure vital signs and weights are being performed as ordered by physician for each participant and documentation is completed accurately and in a timely manner.
  • Document participant issues, goals and services provided in an accurate and timely manner.
  • Ensure nursing best practices are maintained, including compliance with WAC requirements.
  • Participate in staff meetings, and provide health and safety trainings as need for staff.
  • Perform other duties as assigned.
Care Planning Management:
  • Participate in the development of individualized care plans
  • Plan, schedule and facilitate care plan meetings with center staff to ensure Care Plan issues and goals are appropriate, that issues and goals are being addressed, and that reporting is accurate and per WAC requirements
  • Document changes on Care Plans per Physician Summaries and notify stakeholders.
Consultation:
  • Provide onsite consultation to the Licensed Practical Nurse (LPN) at the Des Moines center, review LPN’s participant health changes, and provide feedback regarding questions or concerns.
  • Offer insight, training recommendations, and suggestions for maintaining the best quality practices and to ensure compliance with WAC requirements by all staff.
  • Develop and maintain organizational health services policies and procedures, and ensure compliance with WAC, Veteran Administration, and other funding sources requirements.
Nursing Assessments:
  • Review assessments completed by the LPN to ensure accurate and appropriate health care, with a focus on fall risk factors, administration of medication orders and pain, sleep and physical activity results.
  • Oversee the implementation and maintenance of periodic care plan review in accordance with WAC requirements.
  • Provide modeling, coaching, and/or instruction/demonstration to LPN of how to complete and submit documentation, as well as charting practices, to ensure compliance with WAC, VA, or other funding sources requirements
Qualifications:SKILLS AND EDUCATIONAL REQUIREMENTS
  • Current RN accreditation/licensure in WA State
  • 2 years experience in mental health, geriatrics, developmental disabilities or similar fields (minimum)
  • Practical working knowledge of the Washington Administrative Code for Adult Day Health (preferred)
  • Prior experience working with aging adults and people with disabilities (preferred)
  • Ability to work effectively in a team-focused environment.
  • Ability to prioritize and handle multiple tasks in a timely manner, with a positive attitude.
  • Strong written and verbal communication skills.
  • Proficiency in Microsoft Office and patient database programs.
  • Ability to move, lift and transfer participants using proper transfer techniques.
  • Flexible schedule of availability.
  • Current CPR/First Aid Certification, TB Testing and Food Handler’s Permit within 30 days of hire.
  • Must pass DSHS background Check (and FBI fingerprint check if a Washington resident for less than three (3) years.
  • Proof of U.S. Citizenship or right to work.
INTER-RELATIONSHIPS

Responsible for maintaining relationships with many individuals.  Frequent contacts include supervisors, coworkers, participants, case managers, physicians/doctor’s office staff, and family members/care providers on a daily basis.  A professional demeanor and flexible approach are required, as is knowledge of who to refer to when faced with an unfamiliar issue.

WORKING CONDITIONS

Most duties will be carried out at the Bellevue center and on center grounds, with approximately 4 hours per week providing onsite consultation services at the Des Moines center.  Occasional travel to other offices and/or program facilities may be necessary.  Physical requirements include:

  • Ability to lift up to 40 lbs. up to 20 times per day
  • Ability to frequently change position: stand, sit, stoop, kneel, bend, twist and carry
  • Must be highly flexible and adaptable, able to respond quickly to the changing needs of   participants, families and the adult day services program
  • Work hours may vary to meet the needs of the participants and program.
