Assist individuals seeking assistance in navigating available community resources, including assessing their individual needs, searching for services in our community resource database, conducting intakes as appropriate, and providing appropriate referrals. Service is provided primarily by phone but may also be provided by text message, live chat, email or in-person.

Areas of Responsibility: 
  • 75 % Community Resource Navigation: Receive and respond to incoming 2-1-1 calls, texts, chats and emails from individuals, families, and community organizations seeking assistance to address a wide variety of needs. Assesses their individual needs, provides appropriate referrals to available services through the 2-1-1 resource database, and documents all demographic information, unmet needs, and referrals provided.
  • 20% Screening, Intake, and Follow Up: Conduct screenings for a variety of community initiatives (such as early intervention, smoking cessation, cancer prevention, financial empowerment) and promote preventive programs to help improve the individual’s and family’s health, education, income and overall well-being. Conducts intakes for specific programs as assigned. Conducts follow up with clients to see that needed services were received and to provide additional referrals. Conducts advocacy for individuals experiencing difficulties or lacking abilities to make effective contacts with agencies.
  • 5% Special Projects: Communicate resource database changes to the resource staff and assist with community resource database updates. Assist with filing, data entry, mailings, and surveys. Represent agency at events in the community such as health fairs.

JOB RELATIONSHIPS WITH: Coordinator, 2-1-1; Community Impact Department; All internal staff of United Way of Summit County; Staff and volunteers of local Nonprofit Organizations and Government Agencies in Summit County and in the 2-1-1 resource database

Educational Background: 
Bachelor’s degree in Social or Human Services or related field preferred.
Skills/Experience: 
  • Experience in information and referral agency preferred.
  • Ability to obtain the Community Resource Specialist credential from AIRS within the first 12-14 months of employment.
  • Flexible schedule between the hours of 7:00 a.m. – 7:00 p.m. to accommodate business needs. Schedule will be assigned at least two weeks in advance. Schedule will include a rotating on-call week to accommodate emergencies and call offs. Throughout 7 business days, hours will total 37.5 hours per week.
  • Competent in MS Office products.
  • Interest in the improvement of the Summit County community.

PHYSICAL/OTHER REQUIREMENTS:

  • Position is primarily a normal office setting, requiring long periods of sedentary work at a desk and computer.
  • Ability to routinely travel throughout multi-floor office, walking, climbing stairs.
  • Must be able to sit, stand, reach with hands and arms, bend and stoop.
  • Extensive reading.
  • Minimal lifting up to 25 pounds occasionally.
  • Must be willing to work non-standard work schedule including nights, weekends, and other unusual hours on occasion. This includes the on-call shift which is approximately six to twelve weeks a year to cover any shift needed.
  • Must report to work and be willing to work non-standard hours in case of a community wide disaster.
  • After-hours event attendance occasionally.
