If you answered yes, then Easterseals Florida is looking for you to fill the role of Data Support Coordinator. Our program serves children from birth to age 3 with developmental delays. The Data Support Coordinator is responsible for supporting the Early Intervention Assessment team by assisting with data entry, accepting referrals and scheduling evaluations for entry into the Healthy Beginnings system of care.
Areas of Responsibility:
- Serve as the initial point of contact for the Early Intervention Assessment Team, obtaining all referral messages and ensuring callbacks are returned promptly
- Obtaining and processing all incoming referrals from HomeSafe and the Healthy Beginnings Data System
- Completing intakes in accordance with the program guidelines and verifying all required intake data
- Scheduling initial eligibility evaluations and monitoring program compliance
- Accurately create and update electronic charts, documenting all contacts in the records
- Provide data reports as needed for Children's Services Council reporting
- Process appointment cancellations and reschedule appointments
Educational Background:
High school diploma or GED; Associate's degree or equivalent and one year of experience and/or training
Skills/Experience:
- An equivalent combination of education and experience as described above will be considered
- Outstanding computer skills including experience using Microsoft Office products
- Ability to read and interpret documents and write routine reports and correspondence
- Ability to accurately enter data into a computer database system
- An energetic and professional individual who will promote a positive, respectful and team-based environment
