Basic Responsibilities:• Responsibilities:  Individual/Family Related

  • Link and coordinate the individuals to resources in the community to help them to become successful and become more independent. 
  • Maintain confidential records on all individuals, which include individual goal planning, progress toward goals, medical information, and assessments of resident adjustment.
  • Assist individuals with handling money, making and keeping appointments (doctor, dentist, etc.) as needed.
  • Help to provide opportunities for individuals to and from places of employments, personal appointments, and for recreational and other events.
  • Encourage residents to do things independently, make their own decisions, interact with others, and participate in community activities.
  • Maintains awareness of successes, problems, and general attitudes of the individuals, their employers, and the community.

Administrative

  • Maintain program within approved budget
  • Attend treatment team meetings for the individuals.
  • Assist Residential Supervisor in completing all necessary HUD forms to include: move ins, move outs, interims, and annuals re-certifications.
  • Oversee general area (interior and exterior); responsible for maintaining upkeep of property.

Qualifications:

  • A High School diploma or equivalent and 2 years experience working with individuals with severe persistent mental illnesses and/or property management experience is preferred.
  • Ability to manage diverse duties and multiple priorities; ability to maintain effective relationships with families, community agencies, and volunteers; ability to communicate effectively both orally and in writing; a valid North Carolina driver’s license and ability to be insured by Easterseals UCP’s insurance carrier is required.
