Under the supervision and direction of the Manager, Community Services, is responsible for monitoring and managing direct care staff adherence to all documentation requirements associated with consumer programs, activities, program plan implementation. Ensures that programs meet the vocational habilitation and rehabilitation needs of persons served with an emphasis on person-centered services and the primacy of the individuals served right to self-determination.
Basic Responsibilities:
- Monitors the development, implementation and documentation of individual plans for all individuals served.
- Works directly with Community Inclusion Specialists to ensure accountability for the efficient implementation of consumer's programs, daily IP goal implementation and tracking, completion of scheuled IP's, quarterlies, progress reports and behavior programs.
- Conducts group and/or individual in-service trainings to staff on all facets of consumer programs.
- Ensures that behavioral programs for consumers are developed, implemented, and revised.
- Oversees all documentation an materials specific to QSR reviews.
- Works collaboratively with all funding sources and providers of service in promoting Easterseals services.
- Assures full compliance and adherence to all applicable CARF standards.
- Supervises and monitors staff's daily implementation of Eastereseals and DDS policy & procedures.
- Writes one-time funding proposals for approval.
- Collaborates with management team in the expansion/diversification of programs, to include enclave development and individual job placement opportunities.
