To assist clients in accessing services ensuring that Nutrition Admissions and PT/OT scheduling services meet client and organizational needs. Saturday schedules are accurate and complete for all disciplines and staff and client planned time away requests are addressed in order to maintain attendance expectations.

Areas of Responsibility: 

Admissions:

  • Responds to inquiries regarding Nutrition and Feeding Clinic services promptly, not to exceed one business day following initial contact.
  • Collects client demographic information, opens electronic medical record file if new client, enters data in the electronic medical records system, and forwards appropriate intake forms to client via patient portal links.
  • Schedules appointment for services in the electronic scheduling system.
  • Assists client in obtaining prescription, as needed.
  • Notifies Client Account Coordinator of needs for benefits verification by forwarding payor information to be updated in electronic medical records system for each client prior to scheduled evaluation appointment.
  • Ensures all prescriptions, referrals and authorizations are current and entered in electronic medical records system prior to service provision.
  • Contacts client to remind of pending appointment.
  • Obtains client availability for ongoing services from provider or family after each evaluation.  Maintains waitlist and provider availability.  Collaborates with Nutrition Manager to match provider availability with client need. Contacts clients to schedule ongoing services, when appropriate.
  • Maintains accurate waitlist and prepares monthly outcomes for inclusion in Center outcomes by Supervisor, Admissions.

Scheduling:

  • Reschedules cancelled therapy appointments per organizational parameters.
  • Assists clinical services staff with rescheduling clients for benefit time away.
  • Maintains accurate clinical services staff schedules in the electronic scheduling system.
  • Notifies staff and clients of schedule changes and provides appropriate coverage information.
  • Receives and responds to client family inquiries for adjustments to schedules.
  • Maintains accurate records of schedule adjustments.
  • Coordinates Saturday Schedules for all disciplines at the Villa Park Center.
  • Notifies Supervisor of any concerns regarding client or staff compliance with organizational expectations.
  • Generates not less than quarterly Attendance Reports in collaboration with the Scheduling Supervisor for review and action by Directors, OT and PT, and inclusion in Center outcomes reporting system by Director, Quality Services.

Organizational Expectations:

  • Provides lunch break coverage for Reception staff, as requested by supervisor.
  • Ensures all interactions/communications with clients, families, and providers are delivered in a clear, respectful and responsible interactional style that demonstrates support of our consumers.
  • Adheres to Easterseals DuPage & Fox Valley (ESDFV) Code of Ethics.  Complies with all Federal, State and local laws and regulations.  Work is performed consistent with established ESDFV Mission, Values, and Philosophy.
  • Participates in other activities that support department projects and goals as requested.
Educational Background: 
High School diploma required.  Bachelor’s Degree in healthcare-related field, preferred.
Skills/Experience: 
  • Minimum 2 years intake/admissions/scheduling experience in a hospital or medical office setting required.
  • High level of organizational skills and attention to detail.
  • Ability to expedite entry process and client’s access to services.
  • Demonstrated computer proficiency and experience with electronic records, scheduling and/or billing required.
  • Strong interpersonal skills; excellent communication skills.
