Community Impact Manager

We have an exciting position for a Community Impact Manager with our Bay Area Division . This role manages the execution of health impact goals by focusing in the areas of hypertension, diabetes, cholesterol, nutrition, obesity, physical activity and secondary prevention efforts (CPR). The Community Impact Manager effectively integrates population health strategies focusing on collaboration across the teams of Development, Quality & Systems Improvement, Foundation Relations, Advocacy and Communications & Marketing partners, as well as external collective impact partners. Directly oversees volunteer engagement initiatives and account management to ensure optimal customer experience and achievement of defined outcomes. This position can be housed in either our Oakland or San Jose, California office.

Areas of Responsibility: 
  • Support the Community Health Improvement Plan Strategies to advance policy, system, and environmental (PSE) changes. Work can include but is not limited to: the engagement and positioning of strategies to drive health behavior (healthy eating & active living), health indicators (blood pressure, diabetes, and cholesterol) and worksite wellness. CI Manager will support an engagement plan for critical volunteers and strategic alliances to drive PSE's.
  • Manage the implementation of related events, grant projects and healthcare programs in compliance with intended goals and objectives including a comprehensive work plan, budget, contract developments, timelines, policies, and regulatory requirements.
  • Support the development, implementation, and achievement of healthcare strategies; impacting health outcomes among diverse patient populations including blood pressure, diabetes, and cholesterol by driving national initiatives (Target BP, CCC-Cholesterol), Community to clinical interventions (CCC) and emerging best practices. With the Sr. Director and Healthcare Team, partner with FQHCs, Community Clinics, and Healthcare Systems partners to drive healthcare strategies.
  • Effectively build a network of meaningful volunteers and partner relationships to create a sustainable culture of health within the diverse & vulnerable communities. Duties include the recruitment, management, engagement and following-up with collective impact partners, volunteers and supporters of the Division's overall efforts in the organization.
  • Other projects and duties as assigned.
Skills/Experience: 
  • 2+ years progressive experience in public health, community health or equivalent.
  • Demonstrated ability to drive toward and achieve results.
  • Complex project management experience in a highly collaborative environment.
  • Experience recruiting and managing volunteers.
