Basic Responsibilities: • Prepare and researches for various educational modules. • Provides education on the goals for each person supported. • Plans and communicates activities that assist with socialization and recreation. • Observes people supported and documents daily progress on goals. • Provides verbal direction and redirection as needed to people supported. • Ensures that attendance is completely daily. • Assists in developing and following a daily scheduled for the program. • Assist people supported to gain independent living skills. • Assist with meal planning, menu development and grocery purchasing for the PSR Program. • Provides daily oversight, education and support to PSR Program participants on kitchen safety and cleanliness in accordance with the NC Department of Health Regulations. • Provides support and assistance to people supported to volunteer in the community at least twice a month. • Ensures accurate and timely documentation is completed and maintained in accordance with documentation regulations. • Completes the CPTA Transportation documentation for the people serviced. • Schedules and coordinates guest speakers from the community as needed for PSR Modules. • Participates in treatment team meetings for people supported. • Links and coordinates people supported with community resources. • Promotes the development of independent for each person served and ensures each person is connected to their community. • Provides transportation to people supported as needed in the community. • Maintains regular attendance and punctuality, manages use of PTO time and follows all policies and procedures regarding time off. • Follows all ESUCP Policies and Procedures. • Must demonstrate Easterseals valued behavior of Accountability, Commitment and Caring as appropriate to position. • Completes required training within prescribed timeframes. • Performs related duties as requested. Qualifications: • High School Diploma or equivalent appropriately accredited by the appropriate state department of education. • A minimum of 2 years’ experience working with adults with disabilities is preferred. • Required to have a valid North Carolina driver’s license, good driving record and be insurable. • Have reliable transportation and be willing and able to move freely around the area served by the program. • Knowledge of community services agencies. • Demonstrated skills in completing daily progress notes for individuals serviced. • Advocacy and supported employment experience is preferred.Job Function:
