Summary:There is a position available, in the facility-based day habilitation program. This position is a non-exempt, 40 hour positions. Hours worked beyond 40 hours in one week require payment of overtime.
Basic Responsibilities:Provides case management as assigned including development of Individual Service Plans (ISP’s) for Program Supervisor’s approval, completion of daily ISP data recording, and daily incidental documentation. Leads daily participant activities. Provides personal care and supports participants in daily activities in the facility and in the community. Substitutes for Program Supervisor during absences.
Qualifications:A Bachelor’s degree is preferred; equivalent experience will be considered. A valid driver’s license and a good driving record required.
