- Assists individuals with activities of daily activities, home care and maintenance, and transportation
- Participate in implementation of person centered plan
- Provide direct interventions to build socialization, behavior management skills and daily/community living skills
- Maintain documentation, progress notes, and other data as needed, including incident and accident reports
- Communicate with other staff and supervisor on progress, needs, and changes
- Respond to individual safety needs, including reporting significant changes in behavior and health
- Provide high quality customer service, serving as a positive representative for the agency with other professionals, families, and the community large
- Perform housekeeping duties as assigned
- Must be able to bend, and lift at least 50 lbs.
- Must have valid NCDL & pass a background check
Educational Background:
High School Diploma or equivalent.
Skills/Experience:
- One year experience working with individuals with severe and persistent mental illness and dual diagnosis is preferred.
- One year experience working in a group home setting or assisting adults diagnosed with AMI with activities of daily living preferred.
