The Program Planning Coordinator will serve in the LTE capacity to plan for the submission of a successful Americorp Implementation grant serving rural and underserved communities in Wisconsin. The core premise of the Connected Communities Initiative is to make it easier for vulnerable individuals in communities to find the resources they need to overcome episodic and chronic conditions that can lead to economic instability and negative health outcomes. AmeriCorps members will increase access to these programs and others in rural and underserved communities throughout Wisconsin. The Connected Communities Initiative planning grant process will effectively recruit, train, and engage annual cohorts of AmeriCorps members to link individuals and families to the public and private agencies and related services to improve health outcomes and serve as levers to address critical social determinants of health.
ESSENTIAL JOB FUNCTIONS:
- Serve as project lead to coordinate and document planning activities to prepare for implementing the Connected Communities Americorp Program
- Identify prospective rural and underserved communities and host sites to participate in a full Americorp project
- Collaborate with the UWWi team to fine tune program methodology that engages United Ways, community partnerships and 2-1-1 regional centers in program planning
- Conduct network mapping and outreach strategies to leverage stakeholders throughout Wisconsin
- Develop and test strategies to successfully plan for Americorp member placement in 10-20 communities throughout Wisconsin
- Provide and present a statistical analysis of need, readiness and interest to determine target communities
- Ensure planning deadlines are met through regular evaluation, methodology, analytic reports and responsive strategies
- Work with 2-1-1 Regional Centers to conduct a resource scan and gap analysis in targeted communities. Maintain data and log all relevant information in CMS platform
- Effectively plan engagement strategies, including planning program visits in rural and underserved communities in Wisconsin
- Develop a plan for site supervisor and member support
- Develop a logic model to measure program effectiveness based on the principals of improving social determinants of health
- Assure that all activities meet with CNCS compliance guidelines and procedures
- Travel rural and underserved communities to support partner meetings
- Demonstrate a strong passion for planning strategies to connect citizens in rural communities to health and human services.
- 1-3 years program planning, data analysis or Americorp service in a non-profit or higher education institution
Strong analytical skills; familiarity with CMS platforms - Knowledge of the principles and practices associated with program planning
- Has a strong command of all Microsoft Office programs, especially Word, Excel, and Outlook
- Polished oral, written, telephone, and e-mail communication skills; ability to interact with a variety of constituencies, including Board members, partners, staff and other volunteers
- Excellent presentation and meeting management skills
- Ability to develop comfortable interpersonal relationships with community members, existing coalitions and nonprofit partners.
