Coordinate, monitor and assist all persons determined to be eligible for the program with all aspects of their services. Maintain organized, accurate, complete and detailed time sensitive case file records that comply with all organizational and regulatory standards on each person served. Remain up to date and knowledgeable about other resources in the community that might provide individuals with the opportunity to live, learn, work and play in their communities.
Two years of experience in human services, special education, or related field; or have a high school diploma, plus five years work experience with case management for persons with developmental disabilities. Vocational assessment background preferred.
