CLASS Case Manager

Easterseals North Texas is seeking a CLASS Case Manager for the Fort Worth office. Under direction of the CLASS Program Director together with the Vice President of Programs and Services, the CLASS Case Manager provides comprehensive case management services to individuals enrolled in the Community Living Assistance Support Services (CLASS) Medicaid Waiver program, administered by the Texas Department of Aging and Disability Services (DADS).  The CLASS Case Manager consistently meets all CLASS program standards as stated in the CLASS Provider Manual, Policy Revisions/Clarifications, and Information Letter Clarifications.  The CLASS Case Manager is an advocate for the clients assigned to his/her caseload, uses the help of the CLASS Administrative Assistant appropriately, and seeks guidance and clarification from the CLASS Program Director as needed and with a commitment to consistent and quality care for all clients.
