The Easterseals Rehabilitation Center's Community Living Services team enables individuals with intellectual & developmental disabilities to thrive in a home environment. Services are provided based on each person's unique needs and goals.
Areas of Responsibility:
Work as a team leader in clients' homes to provide services & continuity of care while assisting QDDPs with maintaining sites & rotating on-call phone. Position is hourly. .
- Maintain strict confidentiality regarding client, employee & agency information.
- Work approximately 30 hours per week on the floor as a DSP, primarily on second shift, with adjustments to primarily daytime hour while on-call.
- Responsible for monitoring safety of clients assigned.
- Complete incident reports within FSSA/BDDS guidelines.
- Meet weekly at QDDP/Manager meeting to discuss pertinent information with administration.
- Rotate on-call phone with QDDPs/Managers, professionally handling staff questions, call-ins, medical & mediction issues, etc. Calls must be returned within 10 minutes.
- Assist QIDPs with essential responsibilities.
- Complete "activities" portion of monthly summaries & attend client meetings for assigned clients.
- Keep online client demographic information accurate & current.
- Write goals & service objectives at a minimum annually or as ISPs change/update.
- Inform QDDP of any staff issues that require disciplinary action.
- Ensure coordination & consistency of internal & external services for each client (i.e. behavior clinician, case manager, BDDS etc.) including necessary documentation per state regulations (i.e. monthly summaries).
- Prepare, assist, or supervise clients with daily living skills & activities as needed.
- Scheduling: Follow times and directions on schedule; Employees do not have a set schedule; Work varied days & hours based on client needs.
- Transport clients to appointments, shopping & recreational activities.
- Implement behavior improvement plans by following the outlines exactly & properly documenting incidents.
- Keep daily client progress notes through narrative notes.
- Adhere to all state & agency regulations & guidelines.
- Participate in staff meetings, client meetings & in-service training.
- Responsible for contacting supervisor & finding own replacement at supervisor’s discretion.
- Responsible for entering work into online app accurately & within 24 hours of each work segment.
- Responsible for training new staff annually on clients' ISPs and risk plans.
- Responsible for verbally testing staff's competence routinely in clients' risk plans, prohibited interventions, abuse/neglect/exploitation policy, and reportable incidents.
- Train new staff on medication administration protocols (if SDSP cannot).
Educational Background:
Have a Bachelor’s Degree, high school diploma, or GED. Have/Maintain First Aid and CPR certification.
Skills/Experience:
- Complete State Mandatory Medication Administration Training upon being hired.
- Have at least one year of experience working with persons with intellectual / developmental disabilities or complete additional training upon being hired.
- Have a valid driver’s license and insured vehicle.
- Be at least 18 years of age. Requires the ability to hear, see, talk, read, and write on constant basis.
