MaineGeneral Medical Center, Assertive Community Treatment (ACT) is intended to support adults with severe and persistent mental illness toward the goals of recovery, resilience and community based living. The ACT team includes health information specialists, case managers (CMs), psychiatric mental health nurse practitioners, nurses, employment specialists, substance use providers and a clinical counselor.
Areas of Responsibility:
- The Case Manager will use Motivational Interviewing and the Stages of Change, among other modes of care
- Will assist with maintenance of safe housing for clients
- Contribute to on-call support
- Support treatment plan goals which include, among others; health, primary and dental care-coordination, vocation, community access and medication management
- Work with KePro and other aspects of MaineCare
- Complete typical administrative duties associated with Case Management
Educational Background:
BS/BA degree in Social Service field - Preferred
Skills/Experience:
- Driver’s License – Required
- MHRT/C Certification - Required
- Certified Alcohol and Drug Counselor (CADC) -Preferred
- Experience working with Department of Health and Human Services Section 17 consumers and providers – Preferred
