  • Assist individuals and families to link with requested service providers and activities in the home community.
  • Support participants, representatives and managing employers who direct their own waiver services by providing direct assistance toward their responsibilities including ongoing information about the responsibilities of self-direction.
  • Ensure the appropriate development of person centered plans and the implementation of these plans
  • Complete all required documentation to ensure local, state, federal and Agency requirements are met.
  • Collaborate with MCO and other entities to ensure supports are developed based on wishes and desires of individual and family.
  • Ensure necessary authorizations are in place before supports begin or continue. 
  • Promote team and employee interaction and communication. 
  • Maintain awareness of successes, problems, and general attitudes of the individuals, their employers, and the community.
  • Coordinate the scheduling of services and the implementation of supports including coordinating with the Agency with Choice and Employee of Record, if these models are selected by the participant.
  • Be available for families and Service Providers in times of emergencies.
  • Provide advocacy for individuals in all areas of life, when the Individual Service Plan identifies the need.
  • Train individuals on the process of self-direction when the Individual Service Plan identifies the need.
  • Maintain an awareness and presence in the communities being served to identify potential activities or other natural supports potentials for individuals being served.
  • Maintain records in compliance with the MCO and ESUCP’s Quality Management requirements. 
Educational Background: 
High School diploma or high school equivalency (GED). A College degree preferred in the human service field.
Skills/Experience: 
  • A minimum of one year of documented experience working with individuals with the population served.
  • A valid, unrestricted North Carolina/equivalent driver’s license, good driving record and possess current, unrestricted auto insurance.
