Basic Responsibilities:

Responsibilities:  • The Nurse is a member of the Assertive Community Treatment Team (ACTT) that includes a Psychiatrist, Social Worker, Substance Abuse Counselor, Mental Health Nurse, and other professional and paraprofessional staff.  This person is licensed as a Registered Nurse in the State of North Carolina and possesses the clinical skills and experience to assess, plan, develop, coordinate and provide treatment, rehabilitation and support services for individuals.  The purpose of this position is to assist and support supported individuals of  ACTT  in the development of skills, behaviors, responsibilities, and supports needed to live, learn, work, and be integrated into the community with the greatest possible degree of self-determination and independence.  This person will provide nursing services including administration and monitoring of medication, nursing assessments and interventions.  Under the direction of the team psychiatrist, the nurse will develop, revise, and maintain medication protocols, policies and procedures.  The nurse will record and administer psychotropic medications prescribed by the team psychiatrist.  The nurse will evaluate and chart psychotropic medication effectiveness, complications, and side effects.    Essential Job Functions: • Manages the medication system in conjunction with the psychiatrist, administers and documents medication treatment. • Screens and monitors the individual for medical problems and side effects. • Engages in health promotion, prevention and education activities. • With individual’s agreement, develops strategies to maximize taking medications as prescribed. (e.g., reviewing home environment to find cues to remind beneficiary to take medications; work with individual and psychiatric care provider to scale back the number of times medications are taking each day). • Communicating and coordinating services with other medical providers. • Educating the team in monitoring psychiatric symptoms and medication side-effects.  • Functions as a practicing clinician on ACT Team  • Provides structured face-to-face scheduled therapeutic interventions to provide support and guidance in all areas of functional domains: psychosocial rehabilitative interventions, case coordination, adaptive, communication, personal care, domestic, psychosocial, problem solving, etc. in preventing, overcoming, or managing the recipient's level of functioning and enhancing his/her ability to remain in the community. • Makes every effort to meet critical standards contained in the most current edition of the National Program Standards for ACT Teams as established by the National Alliance for the Mentally Ill or US Department of Health and Human Services, Center for Mental Health Services • Maintains documentation as required, including medication administration record (MAR) • Assists in preparing documentation for utilization review • Assists in training new employees • Completes required training within prescribed timeframes • Successfully participates in the DMH approved ACTT training • Must have general knowledge regarding principles of Assertive Community Treatment and the laws, regulations, and policies that govern Assertive Community Treatment.   • Must possess and obtain a thorough knowledge of adult psychopathology, psychiatric nursing practices, medications, and community resources.   • Other duties as assigned

Qualifications:

• Must hold an active, unrestricted license as a registered nurse through the North Carolina Board of Nursing    • Excellent oral and written communication skills. • Valid, unrestricted, state appropriate driver’s license. • Ability to handle multiple responsibilities and assign priorities. • Certification as a medical administrative technician or equivalent preferred. • Six months experience in mental health environment preferred.
