Basic Responsibilities:

• Responsibilities: Advocate for personal outcomes for children and families, adhering to best practices. Coordinate referrals for placements and receive all necessary paperwork prior to service delivery. Coordinate admission and discharge planning with respective contracted agencies including completion of required documents prior to placement and conducting an initial visit with the child within 72 hours. Supervise and monitor service delivery through home and community visits assuring services meet or exceed state, local and ESUCP standards. Assist Licensing Coordinator with recruitment and provision of training of prospective foster parents. Support children, families, foster parents and contract agencies by responding to calls promptly. Communicate with collateral agency representatives regarding child/family status including medication changes, residential situation, staff changes, school performance, incidents, etc. (within 24 hours) Assure adherence to Client Rights and Confidentiality Rules including HIPAA regulations. Attend community appointments as needed. Provide direct supervision and support to Therapeutic Foster Families. Visit each Treatment Family weekly to provide face to face supervision/guidance and to oversee treatment. Provide additional required training to Treatment Parents prior to placing an Intensive Alternative Family Treatment level child in the home. Make daily contact with Treatment Parents for Intensive Alternative Family Treatment level children.  Prepare and maintain service records on children and families in accordance with NC DSS Administrative Rule 10A NC 70 and DMA documentation standards. Review documentation to ensure that all goals/objectives as outlined in service plan are worked on and documentation meets Medicaid guidelines. Complete paperwork accurately and timely and files it by the 10th of each month for the month prior. Ensures all required electronic data is complete, correct, and entered into Client Care Web within required timelines for Intensive Alternative Family Treatment level children. Coordinate and oversee initial and ongoing assessment activities. Facilitate person centered planning and child & family team meetings. Develop initial and ongoing revisions of person-centered plan and monitor the implementation of person-centered plan. Link children/adolescents/families to additional services and arrange for referrals. Complete application forms to access needed services in the community. Assure authorizations and re-authorizations are submitted accurately by due dates. Document services according to ESUCP, state and federal guidelines. Keep supervisor informed of clinical and administrative issues relating to service delivery. Participate in on-call rotation for routine and emergency situations. Respond to emergency/crisis situations as needed during the day, night, holidays and weekends. Assure that all required billing documentation is prepared accurately and forwarded by due date to appropriate agency/department. Assist program in maintaining sustainability by following protocols for staying within budget.  Transport individuals being served, as needed. Troubleshoot for treatment teams when issues or challenges arise. Ensure incident reports are completed and filed within the 72-hour deadline. Attend and participate in required trainings, seminars, workshops. Maintains a caseload of 5-7 IAFT, or 9-12 TFC children (or combination thereof) for a FT position.  Follows all ESUCP procedures as appropriate to position. Perform other duties as assigned by supervisor.

Qualifications:

• Qualifications:  • Bachelor's degree in social work from an accredited school of social work; or • Bachelor's degree in a human services field from an accredited college or university plus one year directly related experience; or • Bachelor's degree in a non-human service related field from an accredited college or university plus two years directly related experience.   *Directly related experience is defined as human services experience in the areas of case management, assessment and referral, supportive counseling, intervention, psycho-social therapy and treatment planning.  • Valid North Carolina driver’s license and ability to be insured by Easterseals UCP’s insurance carrier.  • Must have reliable transportation. • Minimum 1 year experience in the mental health or social services field
