Basic Responsibilities:- Process requests for services by obtaining necessary assessments, conducting intakes, completing social histories and determining consumers eligible for services.
- Facilitate the Interdisciplinary Team in developing the Individual Comprehensive Plan.
- Advocate and/or assist clients in receiving services which were identified in the planning process.
- Assist clients, their families and guardians in maximizing their abilities for self-direction by enabling and empowering them to the greatest extent possible.
- Coordinate the implementation and monitoring of the plans.
- Mediate conflicts between providers, parents, clients and/or the agency relative to the identified needs of the client.
- Complete required written reports and paper work.
- Initiate and complete the termination and transfer of an individual when services are no longer required or desired.
- Assist in coordinating community resources identified for the client, family members and/or providers.
- Assist Service Coordination Consumers by providing some direct support provision of services.
- Participate in professional development opportunities.
- Complete required written reports and paper work.
- Able to differentiate between different disability populations, rules and regulations, and funding streams.
- Display good time management and prioritization skills.
- Bachelor’s degree from an accredited college or university in a human services field, and at least one year of experience in the delivery of services to the populations they serve.
- Must present proof of college transcript prior to successful completion of 90 day orientation, highlighting at least 30 semester hours of human service related classes. Transcript must be sent from college directly to Easter Seals Human Resources.
- Ability to communicate effectively in written and verbal formats.
- Certified in Relational Mandt training or participation in Positive Behavioral Supports Training
- Ability to successfully complete Mandatory Reporter of Child and Dependent Adult Abuse within first 30 days
- Ability to operate a computer and navigate through Easter Seals database. Experience in word processing software; Word, Excel, and Access preferred.
- Ability to work with a variety of people, with and without disabilities.
- Access to telephone for on-call system.
- Possession of valid Iowa Driver’s License and maintenance of good driving record.
- Insurable under Easter Seals vehicle insurance policy
