The Weld’s Way Home Coordinator facilitates the implementation of Weld’s Way Home: A Strategic Plan to Address and Prevent Homelessness in Weld County, in order to decrease homelessness and housing instability through a continuum of aligned services. The Coordinator also focuses on and aligns the work of Weld’s Way Home with that of the Household Stability Collective Impact group, which focuses on prevention of homelessness and general household stabilization efforts. This position includes a variety of responsibilities addressing the needs of four populations: families, individuals (chronically homeless), veterans, and youth all while aligning with housing first principles. With other United Way staff, this role works alongside the Northern Colorado Continuum of Care, which is a similar regional effort that includes both Larimer and Weld counties. While an ongoing position, the nature of the work will change as projects are completed.
Two Areas of Program Emphasis – addressing homelessness and preventing homelessness
The Weld’s Way Home strategic plan includes ten areas of emphasis for upcoming program years, all to which this position will either contribute or be the lead facilitator in developing ongoing systems and solutions:
- support of and coordination with Housing Navigation Center efforts and it’s Coordinator,
- sustainable solution for Weld County cold weather overflow shelter,
- coordinated information sharing, including a shared intake process and database,
- community education,
- street outreach for persons not in shelters,
- new affordable housing units tied to long-term case management, 7) expanded case management services,
- discharge planning,
- homelessness prevention and
- homelessness diversion.
The Household Stability Collective Impact planning group is currently drafting a plan for coordination of work and investment along the household stability spectrum of “homeless-->unstable-->stable-->self-sufficient.” This staff person will align their work with priority areas determined by this group.
Development
Keeping in mind four unique populations—families, individuals (chronically homeless), veterans, and youth— and housing first principles, implement the Weld’s Way Home strategic plan and Household Stability Collective Impact efforts.
- Facilitate the Weld’s Way Home Executive Council, a group of volunteers and service providers responsible for plan execution
- Facilitate, as it makes sense (determined by UWWC and the planning group), the ongoing work of the Household Stability Collective Impact group
- Catalyze, monitor implementation of and measure efficacy of the Weld’s Way Home strategic plan and Household Stability Collective Impact efforts
- Create and implement new programs as gaps in homeless and housing services are identified in committees
Research and Connection
- Through relationships, reports, networks, and other means, discover for Weld’s Way Home and Household Stability Collective Impact efforts ways to make them sustainable.
- Meet and collaborate with local, state-wide and national networks, focused funders, governmental officials, nonprofit organizations, innovative businesses, and others that are addressing homeless and housing instability in Weld County.
United Way of Weld County General
- Support and promote the mission, values, goals and objectives of UWWC, including representing UWWC in professional organizations and the community (as assigned)
- Support fundraising efforts of UWWC and activities that support UWWC
- Support UWWC annual events, UWWC disaster response, and other activities as assigned
- Attend weekly staff meetings and in service opportunities

Salary: $31,000 to $40,000 per year
