The primary function of the Targeted Case Manager position is to provide Case Management services for persons with intellectual disabilities and/or mental retardation.
Areas of Responsibility:
- Complete and submit assessments, Support Plans and signed consents in a timely manner for individuals on assigned caseload.
- Facilitates community resource availability based on each individual’s needs to assure maximum community participation.
- Monitors Person Centered Plans follow-up and works with support providers to make necessary improvements in supports provided.
- Maintain a minimum of one face-to-face contact per month with people on caseload and minimum of one contact per month with parent and/or guardian of people on caseload.
- Demonstrate positive team behavior to promote a team-oriented environment and maintain positive external and internal customer service relationships.
- Provide crisis intervention for assigned caseload.
- Continually improve processes and relationships with people on assigned caseload
- Participate in agency training and management meetings as assigned.
- Complete and maintain case management notes as required by licensing entity and Easterseals Capper Foundation (“ESCF”).
- Support the purpose and practices of ESCF and participate in organizational activities.
- Follow organizational policies.
- All other duties as assigned.
Educational Background:
Bachelor of Science degree and/or;
Skills/Experience:
- Two (2) years’ experience working with people with disabilities and/or;
- Full-time experience in the field which may be substituted for the degree at the rate of six (6) months of experience for each semester.
- Valid Kansas drivers license and possess reliable vehicle.
- Must be able to pass background checks and drug test per agency procedures.
