The LSP Care Coordinator is responsible for providing information, referral, advocacy, and case planning design for LSP crisis and vulnerable customers. The primary goal is to provide education and resources to help LSP customers remain successful in the LSP program and become financially self-sufficient.
Areas of Responsibility:
- Connect daily with customers to develop and implement custom self-sufficiency plans.
- Design client specific resources to assist in removing barriers. Conduct follow-up after 6 months. Document and calculate all movement on the self-sufficiency scale.
- Identify and remove barriers to self-sufficiency. Design referrals to assist LSP customer with achieving their self-stainable goals.
- Document all customer action steps and record all referrals.
- Provide validation support for agency submitted LSP applications.
Educational Background:
A high school diploma or associate's degree with some social services coursework
Skills/Experience:
Demonstrated Abilities:
- Ability to handle stressful situations with calm and courteous demeanor
- Strong organizational, interviewing, and interpersonal skills
- Ability to work with a diverse group of individuals from various social-economic status
- Knowledge of government, nonprofit, public health, and/or human service resources
Acquired Knowledge:
- A minimum of 1-3 years of experience in a call center or similar work environment
- Proficient with Microsoft Office applications and Internet usage
