The Transition Management Services Qualified Professional provides services to individuals participating in the Transition to Community Living Initiative(TCLI). TMS QP provides provides rehabilitation service intended to increase and individual’s ability to live independently and to maintain that independence. The TMS QP focuses on coordination of services, referrals to outside agencies for services needed to maintain independence such as primary care, community mental health care, psychosocial rehabilitation programs, and other community integration services. The TMS QP also works directly with the TMS Team Lead to coordinate intakes of new indivduals referred to TMS, as well as completing Person Centered Plans. The TMS QP will also coordinate applications for financial benefits such as food stamps and Special Assistance In Home payments, The TMS QP also focuses on increasing the individual’s ability to live as independently as possible, managing the illness, and reestablishing his or her community roles related to the following life domains: emotional, social, safety, housing, medical and health, educational, vocational, and legal.
Provides coaching, mentoring, and consultation to the individual served to promote recovery, self- advocacy, and self-direction.
- Two year’s experience and must have QP status. Have the knowledge, skills, and abilities to successfully provide TMS to adults with mental illness and substance use disorders, Minimum of two years’ experience working with the Adult Mental Health population. Excellent oral and written communication skills. Ability to handle multiple responsibilities and assign priorities. Must have current, unrestricted, state-appropriate driver’s license. Must have current, unrestricted vehicle insurance and provide own transportation for use in fulfilling job requirements.
