Housing Navigation Center Coordinator

The Housing Navigation Center (HNC) is a physical location where those experiencing homelessness are helped to regain housing and those at risk of homelessness are prevented from losing their housing. As part of Weld’s Way Home, Weld County’s initiative to address and prevent homelessness, the HNC provides clear access to housing services ranging from homelessness prevention to temporary cold weather overflow. This role will require working with a Housing First focus to: support multiple agencies operating in the HNC, assist contracted partner with management of the cold weather shelter, implement best practices for preventing homelessness, expand the capacity for services currently offered, create infrastructure to support expanded services, and create sustainable funding sources for the HNC. The position participates in the Northern Colorado Continuum of Care, a regional planning entity that includes Larimer County homeless serving agencies.

Areas of Responsibility: 
  • Coordinating and supporting multiple agencies that have staff working in the HNC
  • Plan for upcoming cold weather shelter seasons, working with partners to facilitate and manage housing-focused shelter practices
  • Assist a contracted partner with managing the Housing Navigation Center cold weather overflow shelter
  • Implementing housing navigation services including diversion, connecting to community resources, and increasing access to immediate and permanent housing options
  • Connecting services to the Coordinated Assessment and Housing Placement System
  • Plan for long term funding sustainability of the HNC
  • Volunteer engagement/recruitment and management in partnership with the UWWC Volunteer Coordinator
  • Increasing housing options through Landlord Support Initiative
  • Focusing services on housing with support while lowering barriers for accessing those services
  • Create data collection tool and monitor data tracking by partners
  • Developing Housing First programs like a risk mitigation fund (flexible funds that are used to get people housed through paying for documentation to paying for fees, deposits, etc.), a peer support program that provides housing stability to those recently housed, or a street outreach team that works to identify those living out of doors and connect them to services
  • Plan for training of partners as well as additional HNC staff around Housing First, Trauma-Informed Care, Mental Health First Aid, and other necessary topics
  • Work with CAHPS Coordinator to navigation available housing options in Weld County
  • Facilitate the Landlord Support Initiative and work to monitor as well as expand the list of partnering landlords working to house HNC clients; this includes making sure that case managers respond to any concerns addressed by landlords
  • Engage community and faith organizations to create flexible funding for diversion practices and rapid re-housing
  • Work with UWWC Volunteer Coordinator to develop corps of community volunteers to help run the daily operations of the center as well as teaching housing related classes
  • Implement and update policies that help guests at the HNC get their mail, take showers, do laundry, and streamline access to documentation
  • Community engagement and outreach including but not limited to giving tours of the space and presentations to interested parties
  • Work with neighbors to address community concerns. This includes but is not limited to local police departments, near-by businesses or residents living in the neighborhood
  • Work with HNC guests to find and provide resources they may need including, but not limited to: Acquiring identification (ID/DL/BC), Completing intakes with guests, Signing guests up to utilize lockers/mail/shower/laundry, Navigating services within the center and in the community
  • Help to coordinate and oversee partners, interns, and VISTAs in the space
  • Order supplies needed in the space and keep an inventory
Educational Background: 
Bachelor’s Degree and/or background in community development, homeless care systems development, health/human services, or related nonprofit experience
Skills/Experience: 
  • Self-motivated and able to creatively problem solve; proven excellent performance standards and quality of work
  • Ability to work with diverse populations
  • Excellent customer service and phone skills
  • Effective interpersonal communication skills including group presentations, training, and one-on-one conversations
  • Above average written communications and public speaking ability
  • Organizational skills, detail-oriented, attention to deadlines
  • Ability to multi-task with constantly changing priorities and excellent follow-through skills
  • Ability to work independently and as a team player
  • Volunteer and/or staff management
  • Data management and analysis skills
