The Interagency family Preservation Director will be responsible for:
- Providing administrative direction, implementation, supervision and quality assurance for the IFPS Program staff for the prevention of out of home placement. IFPS services include: care coordination, clinical services, and plan of care.
- Serving as a liaison between YMCA YFS and other providers
- Assessing and initiate the process of the collection of fees as needed and will be familiar with the data collection systems and all requirements for contract compliance
- Supervise direct service staff and complete administrative duties as needed
- Manage/provide oversight of programs, client records, databases, contracts and agency compliance for client numbers and services to assure compliance with required regulations
- Maintain statistical data and reporting and generate all statistical, narrative and outcome evaluations for the YMCA, the county and others as needed in a timely manner
- Coordinate with all YMCA Youth & Family Services programs in program delivery and maintain quality assurance of services, outcomes and documentation of outcomes
- Facilitate and oversee agency-wide trainings
- Monitor, approve and process expenses to meet budget
- Consult with outside providers when necessary and outreach to schools and community
- Provide expertise on the provision of appropriate clinical services provided to families, especially with regard to mental health and case management
- Oversee the delivery of appropriate and quality care coordination and direct services to families and approve Plans of Care
- Provide supervision of and support to staff, facilitators, specialists, and parent partners adhering to the legal and ethical requirements of their profession as outlined in the code of ethics; provide clinical guidance, weekly supervision, and facilitate team meetings bi-weekly; attend regularly scheduled supervision with the Director of Clinical Services
- Review Care Coordinator progress notes, providing feedback as necessary to ensure fidelity to the wraparound model. Ensure that documentation is completed in a timely manner by turning in approved notes within 24 hours of the note deadline
- Facilitate communication among staff to discuss challenges and find creative solutions to support the families in the implementation of the Plan of Care
- Complete clinical supervisory documentation and notes in a timely manner consistent with deadline for other staff
- Assist staff in preparing and tracking Plan of Care and Safety Plans as well as all required outcome measures/documentation
- Conduct quarterly and discharge chart reviews and provide follow-up to ensure that all required elements of documentation are included in the chart
- Maintain confidentiality in all matters within the organization with respect to personnel, operations and children
- Share on-call duties
- 5-7 years experience
- Five to seven years experience working in community mental health settings serving children and families highly preferred
Salary: $54,000 - $60,000 (annually)
