The candidate will serve as a liaison who is responsible for developing and organizing Child and Family Teams to assess and meet the multiple needs of youth and families who have, or are at risk of developing, behavioral and emotional problems and are at risk of out of home placement, according to the Wraparound process. The Care Coordinator will monitor services provided by other agencies and aid in finding informal supports for families via recreational activities, mentoring and connection with faith community if requested by the family. 

Areas of Responsibility: 
  • Elicits values and incorporates family's culture in developing individualized service coordination planning 
  • Develops with the Child & Family Team a coordinated, proactive crisis/safety plan that addresses immediate and ongoing needs
  • Provides documentation of Child & Family Team meetings to team members and others as needed to support communication, inclusion and accountability
  • Coordinates and manages the scheduling of Child & Family Team meetings
  • Assists the youth and family in accessing strengths-based mental health, social services, educational services and other support and resources required to assist the family to attain its vision
  • Manages a diverse caseload of up to ten (10) clients and ensure that frequency of meetings is responsive to the need for planning
  • Has the ability to work independently, and to meet with families at their available times which may require early morning, evening and/or weekend hours
Educational Background: 
Bachelor’s degree from an accredited college or university in psychology, social work, or related field, with experience in human services, preferably case management; Bilingual (English/Spanish) required.
Skills/Experience: 
  • Experience working with youth and parents in a multicultural community, with diverse populations, collaborating with multiple agencies, and working with the school system.
  • The Care Coordinator must demonstrate excellent written and verbal communication skills and personal values consistent with the YMCA mission.
