The Bilingual Case Worker will be responsible for:
- The bilingual (English/Spanish) case worker is responsible for assessing needs and strengths of families at risk of out of home placement. They will adhere to the principles of wraparound model.
- The case worker is expected to communicate clearly with all family members and to respect and value their culture and beliefs.
- The candidate in this position will be responsible for monitoring services provided by other agencies and for finding informal supports for families from recreational activities, mentoring and connecting with faith community if requested by the family.
- Assess child, youth, and family strengths and needs.
- Gather and ensure the availability of documentation (assessments, reports, concurrent planning) from all involved child-serving systems.
- Assist family in meeting service plan goals.
- Ensure families take ownership for their plan of care.
- Elicit values and incorporate family’s culture in developing individualized service coordination planning.
- Document strengths-based planning by family across all relevant life domains in a Plan of Care.
- Develop a coordinated, proactive crisis/safety plan that addresses immediate and ongoing needs.
- Provide documentation of intervention to team members and others as needed to support communication, inclusion and accountability.
- Coordinate and manage the scheduling of interagency meetings at admission and prior to discharge.
- Provide time-limited, home-based case management services to address factors that place children at risk for out of home placement.
- Provide intensive services of at least 5 hours a week of face to face care coordination for a maximum of 6 weeks with a transition to a step-down level of an average of 2-3 hours a week of face to face care coordination for a maximum of 8 weeks.
- Manage a diverse caseload of up to five families and ensure that frequency of meetings is responsive to the need for planning.
- Ability to work independently, to meet with families at their available times which may require early morning, evening and/or weekend hours.
- LBSW, LGSW, or LCSW certification in the state of Maryland.
- Must have clinical license in good standing with the respective licensing board and must have completed required CEUs and training appropriate to the practice of clinical supervision.
- Bilingual (English/Spanish).
- Experience in working with diverse populations; and skilled at collaborating with multiple agencies(P).
Salary: $46,000 - $53,000 (annually)
