Supported by funding provided by the Administration for Community Living (ACL), the Educator and Care Specialist position will be responsible for providing education, information, referral and support services to people with dementia, caregivers, family members and the community. Additionally, the position is responsible for community outreach, development and awareness activities.   

This opportunity is based in the Ventura Office and is funded by a three year grant term with potential for longer employment.

Areas of Responsibility: 
  • Provide individual and family care consultations and coaching sessions in person, by telephone and via e-mail.  
  • Evaluate individual constituent situations and make recommendations to appropriate services and providers, including related follow up.   
  • Maintain service records and provide reports and statistics as requested.  
  • Manage respite grant program, including applications, home visits and check requests.  
  • Manage support group facilitators, including recruitment, training and coordination.  
  • Provide community consumer education programs in both English and Spanish.  
  • Assist with the marketing and coordination of educational events and other Chapter initiatives as needed.  
  • Cultivate and maintain relationships with community partners and supporters.  
  • Attend community events such as health fairs, forums and seminars.  
  • Partner with other organizations to provide community services, focusing on those serving diverse populations.
Educational Background: 
A Bachelor's Degree in public health, social work, human services or a related field. A combination of education and experience may be considered
Skills/Experience: 
  • A minimum of two years of experience in community outreach, conducting education programs, health care guiding, coaching and/or supporting individuals and their families  
  • Experience working with people with dementia and their caregivers preferred  
  • Knowledge of Alzheimer’s disease, dementia and community resources preferred  
  • Ability to work with a wide range of people including persons with dementia, family caregivers, corporations, foundations and volunteers  
  • Excellent customer service skills when working with internal and external clients  
  • Excellent written, verbal and interpersonal communication skills  
  • Ability to work with diverse communities and demonstrate inclusion  
  • Ability to work independently and as a part of a team  
  • Ability to research, plan and conduct educational programs  
  • Ability to organize, plan, prioritize and carry-out work  
  • Proficiency with Microsoft Office, Google Suite and other work-related software   
  • Ability to multi-task and handle changing priorities   
  • Access to a reliable vehicle, a valid driver’s license and proof of automobile insurance  
  • Ability to work evenings and weekends as needed  
  • Ability to travel as needed to perform job duties  
  • Ability to lift and carry up to 25 pounds  
  • Bilingual English/Spanish required
