The American Heart Association (AHA) has a part-time, grant-supported position available with our Puget Sound Division’s Community Impact Team. This is a great opportunity to gain valuable professional experience and make a lifesaving difference.
Maintaining a healthy blood pressure is key to heart health and this grant-supported initiative expands our ability to empower community members to learn about, monitor and manage blood pressure through a combination of evidence-based resources.
Schedule: 15 hours per week up through March 2020. Flexibility is needed with occasional mornings, evenings and weekends.
Primary responsibilities in supporting the Community Impact team with the management and oversight of this grant funded project include:
- Strategic Planning & Development
- Support Senior Director and Director with strategic planning for achieving grant goals and objectives.
- Collaboratively develop strategic plan for clinic integration of hypertension programs with internal and external stakeholders
- Development of project work plans and goal setting.
- Prepare and create health promotional materials needed for community events, professional health training, patient health education and workshops
- Collaboratively develop an outreach and recruitment plan with internal and external colleagues, partners and stakeholders.
- Initiate opportunities for outreach and promotion of project via social media avenues with Sr. Director & Communications team.
- Project Implementation:
- Lead project implementation efforts with the Community Impact Team, strategic partners, and volunteers.
- Successfully execute clinic hypertension workshops, and Check.Change.Control Blood Pressure programming by March 2020.
- Provide ongoing support and training to health providers, volunteers and community members.
- Partnership Cultivation & Collective impact:
- Cultivate and engage volunteers and community members in supporting the project .
- Coordinate cultivation of collective impact opportunities with local partners within the community examples: hospital, nonprofits, business, community centers, churches, schools
- Build opportunities to leverage outreach, blood pressure awareness, and improve access with partners.
- Evaluation & Reporting:
- Prepare midyear and end of year reporting for internal and external stakeholders.
- Reporting should highlight the qualitative and quantitative results of the project.
- Overall Team Support
- Assist in logistics and execution of training , seminars, and events
- Assist in Community Health Fair and Speaker requests, if needed
- Assist in making event materials/files as it relates to community impact programs
- Identify and recruit volunteers for community engagement opportunities
- Demonstrated skills in written and oral communications, including large and small group presentations, group facilitation and training and writing clear, concise narrative reports and evaluations
- Ability to travel across the communities served
