General Summary

The mission of the Alzheimer's Association is to eliminate Alzheimer's disease through the advancement of research, to provide and enhance care and support for all affected, and to reduce the risk of dementia through the promotion of brain health. Founded in 1981, the Northern California and Northern Nevada Chapter operates out of its main office in San Jose, California with regional offices in 10 other locations.

Under the direction of the Director of Diversity & Inclusion, the Family Care Specialist is responsible for enhancing direct assistance and professional intervention to Alzheimer's families and to foster cooperative relationships with the local community health and social service professionals. The Family Care Specialist must be fluent in Cantonese and/or Mandarin in order to provide direct clinical assistance and support for Chinese families. This position is responsible for the delivery of Helpline and Care Consultation services. The Family Care Specialist is also responsible for presenting educational programs on Alzheimer's and related disorders to the community.

Essential Job Functions

Patient and Family Services (75%)

  • Casework with Alzheimer's families
  • Provide consultation to individuals and families (needs assessment, care planning, resource referral, advocacy as needed, follow up and documentation), in-office, by telephone and, if needed by home visitation
  • Document all case notes in Personify database
  • Monitor cases and follow up as needed
  • Coordinate with health and social service workers in assisting families
  • Document all Helpline intake and follow-up calls in Personify database AlzDirectConnect
  • Responsible for first contact with Chinese clients referred by AlzDirectConnect physician referral program and providing follow-up as needed • Update referral source with services provided
  • Assist with care coordination with health and social service workers
  • Enter client and referral source information into physician outreach database
Outreach & Education Programs (22%)
  • Stays current on Alzheimer's disease (AD) care and research, family caregiver issues and related interventions
  • Present educational programs on Alzheimer's and related disorders to family caregivers, community groups and organizations in Chinese
  • Identify opportunities to collaborate and further develop relationships with community agencies and ethnic community providers to promote awareness of the Alzheimer's Association
  • Represents the Alzheimer's Association at community meetings, events and occasional health fairs
  • Track and report monthly statistics for grant administration: Helpline, Care Consultation, Caregiver Training, Support Groups, Community Education and Outreach Programs
  • Recruit volunteers to support the full mission of the association
Safety Services (3%)
  • Coordinate with MedicAlert+Safe Return Contact Center regarding wandering incidents
  • Assist families with support when a wandering incident occurs
  • Provide assistance to families after missing person is found and provide referrals as needed
CULTURE OF COLLABORATION
  • Fully engage in a culture where team collaboration is highly valued
  • Work as a team to accomplish, if not exceed, organizational goals in the National Strategic Plan
  • Participate in signature fundraising events
  • Walk to End Alzheimer's staff team and personal fundraising goals
  • Work at least one Walk to End Alzheimer's on a weekend
  • Participate in The Longest Day
  • Respond to public policy calls to action & participate in advocacy days
  • Enhance our culture of diversity and inclusion in all aspects of the job

Minimum Requirements

  • Masters in Social Work, Family Counseling, or Gerontology
  • Fluent in Chinese (Cantonese and/or Mandarin)
  • LCSW or MFCC licensure a plus
  • Minimum of 2 years' experience in geriatric and Alzheimer's care required
  • Demonstrated skills in speaking and writing on dementia and caregiver issues
  • Familiar with community services, dementia services, care-giving issues, difficult behaviors, latest research, and the role of education in dementia care
  • Care management and case documentation skills: ability to evaluate, care plan, refer to resources, monitor and terminate cases
  • Ability to respond to emotional concerns of family members
  • Strong public speaking skills a plus
  • Computer: Microsoft Office Suite and database management experience required
  • Excellent verbal and written communication skills
