CYW seeks an experienced, energetic Care Coordinator to join the CYW Clinical Programs team (i.e. Care Coordinators, Therapists, etc.), the Bayview Child Health Center (BCHC) medical team, complimentary care providers (e.g., acupuncturist, biofeedback specialist), and community partners to provide comprehensive, trauma-informed care for patients and their families. Care Coordinators serve as the central point of contact for families, clinicians and other members of the BCHC-CYW multi-disciplinary team.

Areas of Responsibility: 

Duties and Responsibilities:

  • Take a lead role in engaging, interviewing, and assessing patients referred for integrated (medical and behavioral health) care by their primary care provider at the Bayview Child Health Center (BCHC)
  • Develop individualized comprehensive care plans in collaboration with patient/family
  • Collaborate with the multi-disciplinary team to implement the comprehensive plan of care and track patient progress
  • Participate in Clinical Team Meetings and Multi-Disciplinary Rounds (MDR) to provide assistance in identifying and resolving biomedical, psychosocial, and mental health needs of patients and families
  • Coordinate referrals to external providers
  • Maintain careful documentation of all coordination activities within the patient’s Electronic Health Record in a timely manner
  • Provide consultation to primary care providers on matters relating to behavioral health and integrated care
  • Participate in trainings to develop and consolidate the ability to deliver trauma-informed care
  • Other duties as assigned by Director of Clinical Programs
Skills/Experience: 

Candidate Requirements:

  • MA/MSW preferred
  • Minimum three years of experience in social services support to clients/patients required, preferably in a multi-cultural service setting
  • Comprehensive collaboration experience in a team environment
  • Familiarity with Adverse Childhood Experiences (ACEs) and their effects on children and adults
  • Excellent verbal and written communication skills
  • Excellent telephonic etiquette 
  • Self-motivated with effective critical thinking skills
  • Ability to work independently
  • Ability to use all software programs effectively, including MS Suite of services
  • Valid and current California Driver’s License and automobile insurance

  • Position reports to the Director of Clinical Programs
