Physician Outreach Manager

General Summary
The Physician Outreach Manager is responsible for outreach to physicians, ancillary healthcare and service providers for the Alzheimer's Association, San Diego/Imperial Valley Chapter. The position has primary responsibility for conducting outreach to health systems, community clinics, physicians, social workers, and case managers with the goal of developing mutually beneficial direct referral relationships. Measurable outcomes are increased referrals from service providers to Association programs and increased Care Consultation delivery.
Essential Job Functions
- Create and enhance referral partnerships with healthcare systems, HMO's, community clinics, service providers, physicians, social workers, case managers and ancillary healthcare.
- Regularly communicate with service providers about the Association's services and Care Consultation through written communication, selected 1:1 meetings, and assuring a presence at professional or key consumer gatherings
- Educate service professionals about the Association's services, importance of early intervention, and Direct Connect Rapid Referral program through speaking events and presentations.
- Assure accuracy of internal professional and referral databases with administrative staff; assure updated information is input into the Community Resource Finder
- Assist in organizing educational seminars and healthy brain initiatives within healthcare community
- Coordinate appropriate partnerships and outreach within rural and under-served healthcare organizations and community clinics
- Assist with maintaining Personify database of physician contacts and external partners in alliance with the Alzheimer's Associations mission
- Develop ways to obtain and sustain relationships with FQHC's, Public Health Departments and small rural health centers/hospitals with healthcare staff and the ability to refer Physician (nurse practitioner) outreach to Director of Medical & Research Initiatives
- Cultivate and recruit supporters for chapter fundraising events like Walk to End Alzheimer's and the Longest Day.
- Attend chapter meetings and special events as scheduled.
Minimum Requirements
- Bachelor's degree with 3 years post graduate work experience, recommended. Background in social services, sales, marketing and dementia with knowledge about the goals of the Alzheimer's Association helpful.
- Demonstrated professional networking / relationships management skills and experience
- Proficiency with Microsoft Office applications including Word, Outlook and strong ability to use Excel. Experience with Access database application preferred.
- Strong organizational skills, detail oriented and ability to plan, coordinate and manage budgets and programs
- Effective written and verbal communication skills
