The Wounded Warrior Project (WWP) Warrior Complex Case Coordination (C3) Manager oversees the Warrior Complex Case Coordination team and ensures the rapid, coordinated, and collaborative response and connection to appropriate care for warriors in crisis and dealing with difficult, complex, urgent issues that exceed the current capability or scope of a specific WWP program.

Areas of Responsibility: 
  • Manage a team of Complex Case Coordination (C3) liaisons, mentoring them and developing their skills and abilities. Promote a supportive working environment where employees are encouraged to proactively address issues and solve problems. Manage team and individual performance to achieve goals.
  • Oversee C3 liaison team as they coordinate warriors’ support and care through one or more internal WWP programs and/or with external organizations, e.g., Veteran’s Administration (VA) and social services agencies, to address various levels of warriors in crisis.
  • Ensure continuity of care through appropriate resources (internal and external) is identified and coordinated for warriors dealing with complex, urgent issues requiring immediate intervention.
  • Ensure referrals and cases of warriors in crisis are responded to and initial assessments and responses timely and appropriately determined.
  • Provides assistance with coordination of warriors’ travel and appointments as needed.
  • Obtain Release of Information from warriors (or from warriors’ families) as required to enable the warrior’s continued long-term coordination of care and treatment and recovery
  • Respond to issues escalated by teammates and C3 liaisons and advise as to appropriate actions. Escalate problems and issues to leadership as necessary to ensure warriors are receiving proper and timely assistance, treatment, and care.
  • Continually follow up with C3 liaison team throughout warriors’ care, treatment, and recovery and serve as a resource and subject-matter expert.
  • Refer warrior and/or warrior’s family to internal resources (WWP programs) as well as external resources, e.g., VA, to meet the needs of warriors and facilitate the recovery process.
  • Oversee coordination of appropriate step-down treatment and resources to ensure warriors receive the assistance needed for their long-term support as they continue their journey to recovery.
  • Empower warriors, in a holistic manner, for lasting recovery by mitigating the challenges that precipitated the crisis.
  • Develop and maintain working relationships with Veteran’s Administration and with local community and government resources.
  • Improve support for warriors in crisis by leveraging strong relationships with internal WWP programs and external resources for the benefit of warriors.
  • Ensure case documentation and data are maintained accurately, timely, and appropriately in database.  Review warrior referrals and cases and evaluate assessments and responses.
  • Prepare briefings and reports for leadership.
  • Other related duties as assigned.
Educational Background: 
Bachelor’s degree in social work, psychology, clinical psychology, rehabilitation counseling, case management, health services administration, or mental health-related field.
Equivalent combination of applicable education, training, certification, or experience may be considered in lieu of degree.
Skills/Experience: 
  • Demonstrated understanding of local community and government mental health resources.
  • Demonstrated understanding of military and veteran issues, and the emotional impact of combat, e.g., brain injury, post-traumatic stress disorder (PTSD), and combat stress.
  • Demonstrated understanding of the challenges faced by warriors affected by trauma.
  • Demonstrated understanding of psychological and mental health issues.
  • Demonstrated understanding of the social perspective of warriors.
  • Exceptional interpersonal skills.  Ability to interact effectively with diverse individuals and build effective working relationships telephonically. Demonstrated emotional intelligence. Ability to respond to sensitive matters with diplomacy and empathy.
  • Demonstrated understanding of relationship management and partnering. Demonstrated ability to develop and maintain relationships.
  • Strong working understanding of WWP programs and other resources available for families and Warriors.
  • Strong written and verbal communication skills.
  • Strong organizational and time management skills
  • Demonstrated computer proficiency using Microsoft Office Word, Excel, Outlook, and PowerPoint.
  • High energy level. Self-starter; willing to take initiative.

  • Five years of staff management or leadership experience.
  • Position requires experience in any one of the following:
    • Five years of experience working with veteran healthcare-focused programs and case management.
    • Five years of experience working with military or veterans’ population in support of their development and readjustment.
  • Must be able to travel between 25% - 50% of the time including overnight and weekend travel

Preferences

  • Master’s degree in social work, psychology, clinical psychology, rehabilitation counseling, case management, health services administration, or mental health-related field.
  • Five years of experience working with local community and government resources providing services for warriors in crisis.
  • Five years of experience working with and providing support to wounded service members who are dealing with both visible and invisible injuries, e.g., brain injury, post-traumatic stress disorder, and combat stress.
