The Alzheimer's Association is seeking a dynamic, motivated and organized individual to provide dementia-related information, referral and support to clients via telephone, e-mail and face-to-face appointments; plan and implement early stage engagement and education activities within assigned service area; and promote the use of these and other Association services through networking and outreach activities.
- Provide Information and Referral and Care Consultation services to constituents via telephone, e-mail, and in person; take Helpline calls on a regular rotation.
- Provide face-to-face Care Consultation services (Care Navigation).
- Plan and implement the Chapter's early stage engagement and education activities within assigned service area.
- Uphold client confidentiality in all interactions: written, electronic, telephone, in-person, etc.
- Enter client information into Association CRM in an accurate and timely manner and follow all Association guidelines for record keeping.
- File all reports in an accurate and timely manner.
- Comply with all Association and departmental policies and procedures.
- Promote Care Navigation and other Association services at networking meetings and via private meetings with potential referral sources.
- With other members of clinical team, plan and implement quarterly in-services for Care Navigation and Helpline staff.
- Speak at conferences or to professional or community groups as needed.
- Facilitate early stage support groups as needed.
- Experience: 2-3 years' clinical experience
- Licenses and Certification: Professional license preferred, but not required.
Skills and Abilities:
- Requires knowledge of general practices of counseling, social work or a similar profession, ideally with an emphasis on dementia, aging, and/or family dynamics;
- Critical competencies include but are not limited to: excellent written and verbal communication skills, relationship management, and time management; ability to work well in a team environment;
- Comfort with public speaking.Â
Travel: Frequent within the specific geographic area; automobile with valid driver's license and insurance required.
Work Schedule: Monday-Friday; ability to work evenings and weekends as needed. Must be able to work weekends during Walk to End Alzheimer's season (September & October).
The ideal candidate will be a highly organized, motivated self-starter with who can work independently, has a strong attention to detail and has the capacity to manage multiple parallel projects or assignments.Â
