Works directly with families requesting respite scholarships and respite providers to coordinate this service for families in the 10-county region. Coordinates other support services for the community including transportation services to attend programs and environmental and safety assessments with Occupational Therapists.
This position may be located at any of the Alzheimer's Association chapter offices across the Rochester & Finger Lakes region.
Areas of Responsibility:
- Works closely with Care Coordinators in the Care & Support Department to process respite requisitions from families applying to the respite scholarship program.
- Works with partner respite providers across a 10-county area to secure respite for scholarship recipients.
- Works directly with families and providers on any requested scheduling changes and/or cancellations.
- Delivers care consultations and information & referral services when working with families as deemed appropriate.
- Tracks and reports all data associated with the coordination of the respite scholarship program including but not limited to constituent information, provider information, scheduling details and billing details.
- Leads the respite committee consisting of scheduling quarterly meetings, sending calendar invitations, preparing agenda and completing meeting minutes, acting as committee facilitator during meetings, reporting progress and questions as necessary.
- Responsible for making recommendations for program improvement and quality assurance to enhance the constituent experience and efficiency of logistics for Programs & Services staff including evaluation surveys administered to constituents accessing the respite scholarships.
- Responsible for ensuring all respite contracts/agreements with providers are approved by the Alzheimer's Association's legal department and are filed, accessible to the agency and current at all times.
- Prepares and presents respite reports to the Programs & Services team consisting of (but not limited to) hours used, families served, types of respite accessed, counties served and total expenses for the period.
- Submits invoices to the finance department for services rendered for respite, safety and environmental assessments and transportation services on a timely basis.
- Enters, monitors and reports all data for the services listed above in the constituent management system.
- Assists with the promotion of trainings offered by the education team targeting respite providers.
- Provides receptionist office coverage as needed including a daily rotation.
- All other duties as assigned.
- Assists at agency events as needed.
- Support for Programs and Services Department including large conferences and events, with some weekend and/or evening hours required
- Assists with program registrations as needed.
- Assists with calling constituents to make personal invitations to programs as needed.
Educational Background:
Bachelor's Degree in human services related field, 2 years' experience preferred.
Skills/Experience:
- Ability to effectively communicate with healthcare providers and families impacted by Alzheimer's and other related dementias.
- Proficient in; Microsoft Windows 7, Internet Explorer or Google Chrome, Gmail, Google Drive applications, client management systems, database management and spreadsheets
- Excellent interpersonal skills.
- Excellent written, oral, organizational and communication skills.
- Ability to plan and prioritize assignments within responsibilities of the position.
- Ability to work independently a must.
- Must be self-motivated.
