Reporting to the Senior Director of Programs & Services, the Care Consultant serves as a key staff person in the delivery of patient and family services. In addition to assessing family needs, helping secure services to support care at home, providing one-on-one education and guidance, and coordinating or overseeing social engagement programs, the Care Consultant plays an important role in the Chapter’s education program, conducting caregiver training and in-services for home health agencies, residential care facilities and other groups. The person in this position also serves as the key staff person overseeing the development, implementation and evaluation of early stage services in the chapter territory, including early-stage support groups and joint engagement activities.
This role is located in Poughkeepsie but covers Putnam, Orange and Dutchess counties.
- Conducts needs assessments for receptive caregivers: increases caregiver knowledge of Alzheimer’s disease and of community resources
- Collaborates with other service professionals to assist families in accessing services that enable them to care for an individual with Alzheimer’s disease or a related disorder at home
- Maintains statistical data regarding Care Consultations and submits quarterly reports of activities; reassesses family needs periodically
- Conducts educational programs for family caregivers and members of the general public, and provides training for CNA’s, personal care aides, and other staff working with individuals who have Alzheimer’s disease or a related disorder
- Promotes the organizations mission through partnerships with other organizations and active participation in local collaboratives and committees.
- Works with other program staff to develop, implement and evaluate a comprehensive early stage program
- Recruits early-stage individuals and caregivers for the program and collaborates with other program staff to provide initial assessments of persons with the disease and their caregivers, to ensure the appropriateness of participation in the early stage program
- Organizes engagement programming for participants with support from program staff
- Works closely with the Director of Government Relations to engage interested early-stage individuals in Advocacy activities
- Collects and analyzes data related to program goals and evaluates the program’s effectiveness
- Participates in early stage-related conference calls with the National office

- Training and experience in the delivery of services to families coping with dementia
- Experience in curriculum development or grant writing a plus
- Experience in conducting training and informational presentations
- Knowledge of community resources
