Under the supervision of the Director of Programs, the Senior Care Consultant is part of the program team and has primary duties in overseeing clinical care, care consultations, support groups, safety and respite services of the Association. As Senior Care Consultant, the role directly supervises the work of Care Consultants on the Care Consultant team. In addition, the Senior Care Consultant will provide clinical supervision and clinical professional development to the Care and Support Program team.

Care consultants are responsible for delivering programs and services to meet the needs of persons with Alzheimer's disease and dementia, their families and caregivers, professionals, paraprofessionals and community members in accordance with the Alzheimer's Association Strategic Plan and Common Program Portfolio.  The person acting in a Senior Care Consultant role provides information, support and problem solving counsel to individuals living with Alzheimer's or a related dementia, their families and caregivers including priority diverse populations among Latino, African American and LGBT constituencies. As such, the Senior Care Consultant plays a critical role in reaching and serving the underserved communities across the five boroughs. As a trained professional, Senior Care Consultant works with individuals or families, in person or over the phone, who have care or dementia related questions by providing problem-solving strategies, disease education, decision-making support, crisis assistance, guidance during transitions and connections to local services. Senior Care Consultants are also responsible for managing support groups throughout the five boroughs

Areas of Responsibility: 

The function of the Senior Care Consultant is to advance the Care and Support arm of the Association's mission. Under the supervision of the Director of Program, the Senior Care Consultant has decision latitude and demonstrated expertise in delivering designated clinical services:

CLINICAL CARE

  • Responsible for overseeing a team of care consultants and assuring that the team meets service provision goals and maintains a high quality of care.
  • Manage the provision of care consultations by triaging Helpline calls, assessing constituent needs and providing support, information, problem solving techniques and planning for future needs.  This may also include the provision of resource materials to constituentsand developing tailored strategies and tools to ensure that services are provided in a high quality, culturally competent and compassionate manner.
  • Oversee the development of a robust program of support groups that serve the diverse needs of the New York City community. Including recruiting, training, supervision and coordination of a professional and volunteer workforce to lead support groups within the 5 boroughs
  • Develop policies, procedures and processes to deliver safety services, respond to caregivers in crisis and assess caregiver needs for respite services.
  • Educate constituents on Alzheimer's disease or related dementia, care giving strategies, communication skills and dementia related behaviors. Education may be one on one or group education. Link constituents to emergency, chapter and/or community resources.
  • The Senior Care Consultant will also oversee the work of Care Consultants and as such will provide clinical supervision, oversight, guidance and assistance to Care Consultants as they conduct their clinical, program development, relationship management, quality assurance, monitoring and reporting tasks.

PROGRAM DEVELOPMENT

  • Develop professional development and educational opportunities for care consultants  and programmatic staff that will help staff maintain a high quality of care including developing a strategy for clinical supervision including planning and leading clinical case reviews.
  • Responsibility to increase and coordinate development and implementation of clinical services to diverse communities and populations in the region.
  • Conduct outreach and education with physicians, builds working relationships with service providers, care facilities and other stakeholders who collaborate on Association initiatives.
  • Remains knowledgeable about current in information related to Alzheimer's disease and related dementias as well as national, state, and local initiatives that affect the targeted population

RELATIONSHIP MANAGEMENT

  • Proactively and  collaboratively work with chapter staff to support the Alzheimer's Association's vision,  mission, core values and strategic goals and promote Association programs such as Advocacy, TrialMatch, The Longest Day, Walk to End Alzheimer's and other Alzheimer's Association special events.
  • Identify business opportunities; strengthen relationships with key individuals, organizations and businesses. Build relationships that will enhance client services and assist in growing service contacts and referrals.

QUALITY ASSURANCE, MONITORING, REPORTING

  • Oversee and assure that  all legal and other requirements with respect to patient records and documentation are completed by the care consultant team and meet Association standards for quality services.
  • Analyze, collect and enter relevant and appropriate data for grant reporting, measuring progress toward strategic goals, and submitting monthly reports to the Director of Programs.
  • Act as a liaison between Chapter, National Alzheimer's Association, and grantors and coordinate systems for collection and reporting of data and outcomes; work with team to meet performance outcomes.
Educational Background: 
Licensed clinical social worker (LCSW) or equivalent with demonstrated experience providing clinical care, mental health, counseling and related services.
Skills/Experience: 
  • Successful experience supervising a clinical team, meeting goals/metrics for service delivery and assuring clinical quality assurance.
  • Familiarity with Alzheimer's and related dementias, and/or aging and health preferred.
  • Experience as a manager or supervisor of staff.
  • Experience facilitating support groups, providing education and or volunteer management.
  • Demonstrated experience working with diverse communities including Latino, African Americans, and LGBT.
  • Excellent written and oral communication skills.
  • Proficient in Windows and other software programs: Word, Excel, PowerPoint, Publisher, Personify and Google Apps
  • Bi-lingual highly preferred (Spanish, Haitian Creole, Chinese, Korean, Russian etc)
  • Ability to work early evening and weekend hours as needed.
  • Valid driver's license, access to reliable vehicle, and proof of current automobile insurance

  • Desire to help families cope with Alzheimer's disease and related dementias.
