Community Outreach Specialist

As part of the Care and Support team, this full-time position will provide outreach to individuals and families dealing with a dementia as as assist with Chapter initiatives in 18 counties in the Kansas City metro area (see list below). Responsibilities include deploying a community engagement and education strategy with emphasis on reaching diverse populations; providing direct support through care consultations, disease education and resources to individuals with Alzheimer’s disease and related disorders, their caregivers and community members; and reaching out to physician groups to build awareness of chapter resources. Services are provided in keeping with the mission, strategic plan goals, national standards and guidelines of the Alzheimer’s Association. This position requires teamwork and collaborative efforts with staff, volunteers, and community partners.

Areas of Responsibility: 
  • Lead efforts to serve individuals and organizations representing diverse populations including African-American, Hispanic and LGBTQ populations in the 18 country KC metro area
  • Provide care consultation, care planning and information and referral to individuals and families dealing with dementia
  • Provide accurate and timely documentation and follow- up of consultation interactions and interventions using Association database
  • Recruit and deploy volunteers to provide education programs, program outreach, early stage and social engagement activities, and support groups as needed to meet the needs of the region
  • Establish relationships with physicians, medical system providers, care facilities and other community stakeholders to build awareness of Association services
  • Travel to Anderson, Douglas, Franklin, Johnson, Leavenworth, Miami, Wyandotte, Bates, Carroll, Cass, Clay, Henry, Jackson, Johnson, Lafayette, Platte, Ray and St. Clair counties as needed
Educational Background: 
Bachelor’s degree in social work or related field with three years of relevant experience
Skills/Experience: 
  • Effective written and verbal communication skills
  • Ability to communicate and work effectively in a team-based environment with clients, families, staff, volunteers, and community members in person and on the telephone
  • Strong problem solving skills and attention to detail. Proven ability to successfully handle multiple projects and meet critical deadlines while working independently
  • High level of integrity, compassion, initiative, and confidentiality
  • Familiarity with Microsoft Office and/or Google Chrome applications
  • The ideal candidate will have some knowledge of the following: community social work; diverse populations; social, governmental and healthcare systems; advocacy; and Alzheimer’s and related dementias.
